Platelet-rich plasma (PRP) for the treatment of vulvar lichen sclerosus in a premenopausal woman: A case report

Abstract: The use of platelet-rich plasma (PRP) for the treatment of lichen sclerosus (LS) in a 38-year-old premenopausal woman is reported. The diagnosis was confirmed histologically and the symptoms documented using the ICIQ Vaginal Symptoms Questionnaire (ICIQ-VS) and the Female Sexual Function Index (FSFI) questionnaire. PRP was prepared from autologous blood using the Regen Cellular Matrix Kit. PRP was administered twice over two months. Histology at follow-up one month after the second administration showed the ep… Show more

“…[58][59][60][61][62] Outside of one-patient case reports, there have been few studies investigating the use of PRP in patients with biopsy-proven LS. [63][64][65] A 2010 study examined injections of a combination of PRP and fat-derived mesenchymal cells in 15 women with vulvar LS. The authors reported that all 15 patients had complete resolution of pain and symptoms.…”

“…No significant difference in terms of number of cells was observed between male and female (mean n of cells, M = 3.48 ± 7.16; F = 2.92 ± 4.84; P = .812), moreover we confirm for LS patients, as previously demonstrated for healthy donors, 39 that age does not impact on the general outcome of cell cultures evaluated in terms of cell viability, adhesion on plastic surface, and cell proliferation (data not shown). Due to the elevated concentration of growth factors extracellular fraction of lipoaspirates enhances the proliferation rate of different cell populations adipose‐derived MSC, dermal, and epidermal cells increasing the therapeutic potential of autologous graft 40‐47 . The general promitogenic effect measured as the mean stimulation induced in different cell types (Figure 2B) or the specific proliferation rate evaluated separately on keratinocytes, melanocytes, fibroblasts, and AD‐MSCs did not find any significant correlation with treatment outcome measured as clinical score (Figure 3A) or ∆DLQI (Figure 3B).…”

“…Sporadic data in the literature reported that the application of lipofilling and autologous (PRP) are effective for the therapeutic management of LS 18‐23,43‐46 . The simultaneous use of lipofilling and PRP has been previously reported in a single case report and in a study including 15 female LS patients lacking a control group of patients treated only with lipofilling in comparison to the combinatory therapy 18,46 .…”
