(1) Background: A proper assessment of disease activity is crucial for the management of a patient with rheumatoid arthritis (RA). Platelets seem to be involved in joint inflammation pathophysiology. Platelet indices (PIs) are markers of platelet activation, and include platelet count (PC), mean platelet volume (MPV), platelet distribution width (PDW) and plateletcrit (PCT). The purpose of the study was to assess the relationship between PIs and disease activity markers, both systemic (clinical, laboratory) and local (ultrasound, US), in patients with RA; (2) Methods: The study group consisted of 131 consecutive RA patients. The following assessments were performed: joint counts, Disease Activity Score (DAS28), complete blood cell counts, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and US of 24 small joints; (3) Results: Mean values of PIs remained within the normal reference ranges. Values of PC, PCT, PDW were significantly associated with disease activity markers, both clinical (DAS28, joint counts) and laboratory (CRP, ESR). In patients with high disease activity, PC, PCT were significantly higher and PDW lower. PC was positively correlated with Power Doppler US (PDUS) score. In patients with features of RA severity (antibodies positivity, extra-articular manifestations) PC and PCT were positively associated with all US parameters (Grey Scale US, PDUS, Global scores); (4) Conclusions: In patients with RA, PC and PCT may serve as positive disease activity markers and PDW may serve as a negative marker. PIs may be used as reliable, inexpensive markers of RA systemic activity; they may also serve as markers of local inflammation in the joints affected by RA.

Background The overall situation of RNA epigenetics in rheumatoid arthritis (RA) is still unclear. This study aims to investigate the expression level of m6A, m5C, m1A, m7G, adenosine-to-inosine, pseudouridine(ψ) modification-related molecules in peripheral blood of RA. Methods 33 RA patients and 36 healthy donors (HD) were included in this study. Gene expression levels were detected by real time-polymerase chain reaction (RT-PCR). Correlation analyses were performed by Pearson correlation test. Least absolute shrinkage and selection operator regression (LASSO) regression and Logistic regression were used for seeking risk factors. Results The expression levels of METTL3, TET2, ADAR1 increased while ALKBH5, TRMT10C TRMT61B ALKBH3 ALYREF METTL1 decreased in RA patients. Analysis shown 47 weak correlations, 43 moderate correlations and 2 strong correlations existed among these molecules. Area under curve (AUC) of ALYREF and TET2 combined ROC was 0.976 which indicated them were the most valuable risk factors in RA. The expression of ALKBH3 and TRMT10C was higher in ESR-negative RA patients, while the expression of METTL1 and METTL3 was higher in RF-negative group or CRP-negative group respectively. Besides, the level of METTL3 and TRMT10C was increased in DAS-28-ESR. Furthermore, METTL3 level was negative related with the levels of RF and DAS28-ESR. The level of TRMT10C was negative with ESR and DAS-28-ESR levels, and ALKBH5 expression was positive related with CRP level. The differential expression genes were mostly correlated with PLT and MCHC of blood routine examination. Conclusions Our study constructed a network of six kinds of mRNA modifications in RA. We found multiple mRNA-modification-related genes were different expression between RA patients and HD. Complicated mutual relations existed among these genes. ALYREF and TET2 were the most valuable risk factors in RA. This work hopes to support a new perspective for clarifying pathogenic mechanism of RA.

Background: Circular RNA (circRNA) is a type of closed circular noncoding RNA (ncRNA), mostly formed by back-splicing or alternative splicing of pre-messenger RNA (mRNA). The aim of this study was to explore the expression profile of circRNA in peripheral blood mononuclear cells (PBMCs) of patients with ankylosing spondylitis (AS) and discover potential molecular markers of AS. Methods: The circRNA microarray technology was used to detect the expression of circRNAs in the peripheral blood of 6 patients with AS and 6 healthy controls (HC). To screen the differentially expressed circRNAs by fold change (FC) and P value, these differentially expressed circRNAs were analyzed by bioinformatics. In 60 cases of AS and 30 cases of HC, 4 circRNAs were subjected to real-time fluorescence quantitative polymerase chain reaction (RT-qPCR), and their correlation with various clinical indicators was analyzed. Finally, the receiver operating characteristic (ROC) curve was used to analyze their potential as AS diagnostic markers. Results: The microarray results showed that there were 1369 significantly differently expressed ( P < 0.05, FC > 1.5) circRNAs between the AS and HC groups (675 upregulated and 694 downregulated). The results of bioinformatics analysis suggested that they were mainly involved in “enzyme binding,” “adenosine ribonucleotide binding,” “MAPK signaling pathway”, etc . The RT-qPCR results showed that the expressions of hsa_circRNA_001544 ( U = 486.5, P < 0.05) and hsa_circRNA_102532 ( U = 645, P < 0.05) were significantly different between the AS group and the HC group. The AS group was further divided into two subgroups: active AS (ASA) and stable AS (ASS). After analysis, it was found that compared with the HC group, hsa_circRNA_001544 was significantly increased in both ASA ( U = 214, P < 0.05) and ASS groups ( U = 273, P < 0.05), while hsa_circRNA_008961 ( U = 250, P < 0.05) and hsa_circRNA_102532 ( U = 295, P < 0.05) were only significantly increased in the ASA group. Furthermore, hsa_circRNA_012732 was significantly different between the ASA and ASS groups ( U = 194, P < 0.05), and there was no statistical significance among the remaining groups. Correlation analysis results showed that hsa_circRNA_012732 was negatively correlated with Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), high-sensitivity C-reactive protein (hsCRP), and globulin (GLOB) and positively correlated with lymphocyte count (LY), mean corpusular volume, and albumin (ALB), and ...
