The phosphodiesterase 4D gene for early onset ischemic stroke among normotensive patients The phosphodiesterase 4D (PDE4D) gene was recently reported as a risk gene for stroke [1]. Although several studies tried to replicate the results, only one study focused primarily on early onset stroke [2], where the study subjects were Caucasian women and African-American women. Our aim was to study four commonly investigated single nucleotide polymorphisms (SNPs) of the PDE4D gene in young stroke subjects recruited in southern Taiwan. In addition, we stratified the subjects based on the presence of hypertension because two studies reported that the PDE4D genetic effect is more significant in non-hypertensive subjects [3,4]. An interaction between the gene and smoking was also explored in the present study. The patients in the present genetic study were primarily from the Southern Taiwan Young Stroke Study [5]. Our patients included 57 cases of large-artery atherosclerosis, 18 cases of cardioembolism, 66 cases of small-vessel occlusion, 16 cases of other determined etiology, and 33 cases of undetermined etiology. Two hundred and eleven stroke-free participants were recruited from the general population between 2002 and 2006. We selected four SNPs that have been implicated to be associated with stroke: rs12188950 (i.e. SNP45, indicated in the original article [1]); rs702553 (SNP56); rs966221 (SNP83); and rs2910829 (SNP87). Polymorphisms were genotyped using TaqMan Ò technology (Applied Biosystems, Foster City, CA, USA). Each SNP was in Hardy-Weinberg equilibrium for cases and controls. SNP45 was monomorphic in our population.For SNP56, the results indicated that genotypes TT [crude odds ratio (OR) ¼ 2.0, P ¼ 0.014] and AT (crude OR ¼ 1.4, P ¼ 0.157) carried a higher risk than the reference AA genotype (Table 1). It appeared that the T allele had an additive effect, and thus we used the additive model to estimate adjusted OR. After adjusting for significant covariates including diabetes, hypertension, and current smoking, the OR was 1.36 (P ¼ 0.052). The final multivariate regression model suggested that smoking, diabetes and hypertension were the major risk factors for early onset stroke. Further analysis according to the hypertensive status found that the genetic effect was solely from the non-hypertensive individuals, with an adjusted OR of 1.50 (P ¼ 0.020) among non-hypertensive participants and non-significant results for the hypertensive individuals. We did not find an interaction between SNP56 and current smoking. The genotypic and allelic distributions were not significantly different between the patients and controls for either SNP83 or SNP87. Haplotype analysis using the HapClustering program [6] to adjust for covariates did not yield a significant result (overall P > 0.1). For analyses on the stroke subtypes, SNP56 was associated more with the cardioembolic subtype. Association between C807T(C/T) polymorphism of platelet glycoprotein gene and sensitivity to ischemic stroke: a meta-analysis

ABSTRACT. Ischemic stroke can lead to loss of neurologic functions. It occurs due to obstruction in blood supply to the brain. It has been proposed that C807T(C/T) polymorphism within the platelet glycoprotein gene may be associated with density and function of glycoprotein Ia/IIa receptors and contributes to the pathogenesis of thrombotic disease. We assessed the association between C807T(C/T) and risk of ischemic stroke. Databases such as PubMed, Medline, Springer, Elsevier Science Direct, Cochrane Library, Google scholar, Wanfang Data (Chinese), and Chinese National Knowledge Infrastructure (CNKI, Chinese) were used to search for relevant studies. We found 16 eligible studies, which totaled to 4897 (case group 2340; control group 2557) participants. Overall, our results showed significant associations between C807T(C/T) polymorphism and risk of ischemic stroke based on T-allele comparisons (T vs C, pooled OR = 0.78, 95%CI = 0.68-0.90, P < 0.01), TT vs CC comparisons (pooled OR = 0.58, 95%CI = 0.42-0.81, P < 0.01), recessive models (TT vs TC + CC, pooled OR = 0.72, 95%CI = 0.59-0.87, P < 0.01) and dominant models (TT + TC vs CC, pooled OR = 0.70, 95%CI = 0.54-0.92, P < 0.05). There was no association in TC vs CC comparisons (pooled OR = 0.81, 95%CI = 0.63-1.04, P > 0.05). Subgroup analyses stratified according to Hardy-Weinberg equilibrium, sample size, and ethnicity also demonstrated significant associations between the two variables. Therefore, C807T(C/T) polymorphism in the platelet glycoprotein gene may be associated with susceptibility to ischemic stroke, and the T allele at this locus may decrease risk to ischemic stroke.
