Plasmapheresis: An adjunct to medical management of severe hyperthyroidism

Abstract: A woman with hyperthyroidism and myasthenia gravis developed respiratory failure in association with radiation-induced thyroiditis. Treatment with steroids, propylthiouracil, propranolol, iodine, and plasmapheresis was associated with dramatic reduction in serum triiodothyronine (T3), serum thyroxine (T4), and thyroglobulin levels and prompt recovery of the patient. The medications that this patient received have been shown to cause an abrupt decline in serum T3 levels with little or no effect on the serum T4 … Show more

“…However, in patients with iatrogenic thyrotoxicosis or for whom antithyroid drugs are contraindicated or who are resistant to antithyroid drugs, further therapies are needed to restore euthyroid hormone status. The reported cases suggested that therapeutic plasma exchange (TPE) has been used as an additional treatment or preparation for surgery [5][6][7].…”

“…In our case, for example, the best response to TPE was in a patient with a non-autoimmune disease. The success of TPE in this case was attributed to the dilution of hormones and the generation of free proteins with new binding sites, which reduced their free fractions and thus their biological activity [15][16][17][18][19][20][21]. While this is effective in the short-term, its effect is transient and so TPE cannot be used as definitive therapy.…”

“…Plasmapheresis is a medical procedure that removes the thyroid hormones and antibodies from the bloodstream of patients who are in need of urgent surgical intervention. It has been utilized for the treatment of several immunologic and nonimmunologic disorders such as Graves' disease, intoxication of levothyroxin and amiodarone (4,5). Here, we report the case of a patient with toxic multinodular goiter (TMNG) who was prepared for surgery by plasmapheresis due to the side effects of antithyroid drugs.…”
