Plasmacytoid Urothelial Carcinoma

Abstract: The plasmacytoid variant of urothelial carcinoma is rare, of which less than 40 cases have been reported in the English language literature. Herein we report the largest series to date of 17 cases of urothelial carcinoma with plasmacytoid features and report the associated clinicopathologic findings. The architectural pattern of the tumor varied from cells arranged in cords and single cells (35%), small nests (17%), solid sheetlike growth (29%), and diffuse discohesive patternless architecture (23%). The plasm… Show more

“…PUC must be distinguished from other malignancies, such as plasmacytoma, signet ring adenocarcinoma, and metastatic breast or gastric cancer. 13,14 PUC is commonly locally advanced at presentation. [10][11][12] After attempts at definitive local therapy, both pelvic recurrence and metastatic disease are common.…”

“…It was first recognized in the 2004 World Health Organization (WHO) classification of urothelial neoplasms [1]. It is usually diagnosed at an advanced pathological stage, showing metastases in 60% of patients, and has a lower survival rate than conventional UC [2,3,4]. Few reports on the urine cytologic findings of PVUC exist in the literature [5,6,7,8,9], and there is only 1 case report of fine-needle aspiration (FNA) cytologic findings [10].…”

“…An informal review of 23 cases from the literature [2,3,4,6,9,14,15,16] demonstrates that average survival between those receiving chemotherapy versus those without is similar (18 vs. 19 months). This is without taking into account TNM staging.…”

“…The main histologic findings include dyshesive, oval to round plasmacytoid cells arranged in cords, nests, single file or with sheet-like growth. The malignant cells contain abundant eosinophilic cytoplasm, eccentric, enlarged hyperchromatic nuclei, small nucleoli, and numerous mitoses [3,4,5,6,7]. …”
