SUMMARYThe atrial fibrillation (AF) recurrence rate after pulmonary vein isolation (PVI) has been relatively high and in some unsuccessful PVI cases, antiarrhythmic drugs that were ineffective before PVI may become effective (hybrid therapy). The purpose of this study was to investigate the relationship between the brain natriuretic peptide (BNP) level and the effect of the hybrid therapy.In 28 lone AF patients undergoing hybrid therapy, the plasma BNP level was measured before PVI and before and 3 months after administering an antiarrhythmic drug. Fifteen patients were free of AF after the hybrid therapy (effective group), and 13 still had recurrent AF after (noneffective group). At baseline, in all patients the BNP level was elevated, and there were no significant differences in the BNP level between the 2 groups. The BNP level was significantly decreased after PVI in the effective group (56.8 ± 23.1 versus 37.5 ± 16.7 pg/mL, P < 0.05) but not in the noneffective group (74.3 ± 47.8 versus 79.7 ± 54.4 pg/mL, NS). The elevated BNP level normalized in all effective group patients. The criterion consisting of a net value of < 60.0 pg/mL or a reduction in the BNP level of > 10.0 pg/mL after PVI predicted the effective group patients with a high accuracy.A significant reduction in the BNP level after PVI may be a useful predictor of the responders to antiarrhythmic drug therapy in patients with recurrent AF after PVI. The hybrid therapy may be effective in patients whose PVs serve as a dominant AF substrate. (Int Heart J 2008; 49: 143-151)

Фибрилляция предсердий (ФП) -наиболее рас-пространенное нарушение сердечного ритма, встре-чающееся в клинической практике. Появление ФП со-провождается выраженными субъективными рас-стройствами и ведет к ухудшению качества жизни. Наличие у пациентов ФП ассоциируется с приблизи-тельно двукратным увеличением смертности, в значи-тельной степени обусловленной развитием инсульта и прогрессированием хронической сердечной недоста-точности (ХСН) [1].Важную роль в регуляции структурно-функцио-нального состояния сердечно-сосудистой системы отводят системе натрийуретических пептидов (NUP). Предсердный (ANP) и мозговой натрийуретический пеп-тид (BNP) -сердечные гормоны, регулирующие объ-ем жидкости в организме и уровень артериального дав-ления (АД). Ведущим стимулом синтеза и секреции BNP и ANP является повышение напряжения стенки мио-карда. Увеличение внутриполостного давления, объ-емная перегрузка и расширение полостей сердца при-водят к возрастанию уровня NUP [2].В настоящее время накоплено много доказательств важнейшей роли системы NUP в определении сердеч-но-сосудистого риска у пациентов с артериальной ги-пертензией (АГ), ишемической болезнью сердца (ИБС) и ХСН [3,4] Цель. Изучить изменение концентраций мозгового (NT-proBNP) и предсердного (NT-proANP) натрийуретических пептидов у больных с персистирующей формой фиб-рилляцией предсердий (ФП) при лечении различными антиаритмическими препаратами. Материал и методы. В исследование было включено 123 пациента с персистирующей формой ФП в возрасте от 39 до 83 лет. Больные были разделены на 2 груп-пы: 1 группу составили пациенты с артериальной гипертензией (АГ; n=61), 2 группу -пациенты с ишемической болезнью сердца (ИБС; n=62). После восстановле-ния синусового ритма пациенты были распределены на 4 подгруппы. Пациенты 1А подгруппы для сохранения синусового ритма в течение последующих 6 мес при-нимали пропафенон, больным 2А подгруппы для удержания синусового ритма был назначен амиодарон, пациенты 1Б и 2Б подгрупп для контроля частоты желудоч-ковых сокращений (ЧЖС) использовали бисопролол. В первые 3 суток после восстановления синусового ритма и через 6 мес всем больным проводилось исследова-ние уровня натрийуретических пептидов в плазме крови. Результаты. Увеличение NT-proBNP и NT-proANP выявлялось у пациентов c АГ и больных ИБС в первые сутки после восстановления синусового ритма и было обусловлено развитием пароксизма ФП. Через 6 мес лечения отмечалось уменьшение уровня NT-proBNP и NT-proANP у пациентов 1А (NT-proBNP: с 160,5±15,2 пг/мл до 130,1±12,2 пг/мл; NT-proANP: с 15,3±6,4 нмоль/л до 11,5±5,3 нмоль/л; р<0,05 для обоих) и 2А (NT-proBNP: с 180,2±12,7 до 120,6±10,9 пг/мл; NT-proANP: с 17,1±7,1 до 11,8±7,2 нмоль/л; р<0,05 для обоих) подгрупп. Значения NT-proBNP и NT-proANP сопоставимо снижались как у пациентов с АГ при лечении пропафеноном, так и у больных ИБС при назначении амиодарона.
