Background: Shift work-related carcinogenesis is hypothesized to be mediated by melatonin; however, few studies have considered the potential effect modification of this underlying pathway by chronotype or specific aspects of shift work such as the number of consecutive nights in a rotation. In this study, we examined melatonin patterns in relation to shift status, stratified by chronotype and number of consecutive night shifts, and cumulative lifetime exposure to shift work.Methods: Melatonin patterns of 261 female personnel (147 fixed-day and 114 on rotations, including nights) at Kingston General Hospital were analyzed using cosinor analysis. Urine samples were collected from all voids over a 48-hour specimen collection period for measurement of 6-sulfatoxymelatonin concentrations using the Buhlmann ELISA Kit. Chronotypes were assessed using mid-sleep time (MSF) derived from the Munich Chronotype Questionnaire (MCTQ). Sociodemographic, health, and occupational information were collected by questionnaire.Results: Rotational shift nurses working nights had a lower mesor and an earlier time of peak melatonin production compared to day-only workers. More pronounced differences in mesor and acrophase were seen among later chronotypes, and shift workers working !3 consecutive nights. Among nurses, cumulative shift work was associated with a reduction in mesor.Conclusion: These results suggest that evening-types and/or shift workers working !3 consecutive nights are more susceptible to adverse light-at-night effects, whereas long-term shift work may also chronically reduce melatonin levels.Impact: Cumulative and current exposure to shift work, including nights, affects level and timing of melatonin production, which may be related to carcinogenesis and cancer risk. : A new procedure with the G280 antibody of Kennaway provides an assay for circulating melatonin (aMT) with a sample volume (200 μl), an analytic (0.33 pg/ml) and functional (0.62–0.80 pg/ml) detectability, a 50% displacement dose (6.4 pg/ml), a Kd (0.657 pM), and measured circulating daytime levels lower than reported for previous procedures, and 100% assay recovery. The normal daytime range in adult human and Syrian hamster serum was 0.4–4 pg/ml. The pattern of fall of the nocturnal surge of Syrian hamster serum aMT near the time of lights‐on was unaltered by extended darkness. Isoproterenol (ISO) injection 1 hr after lights‐on, when aMT had reached daytime levels, raised serum and pineal aMT dramatically 2 hr postinjection. The same dose of ISO injected 4 hr into light produced only a small detectable increase. Novel extension of nocturnal darkness did not affect the responses to ISO. Thus, when they are allowed to occur at the usual time on a 10‐hr dark schedule, both the fall from the nocturnal aMT surge and the subsequent loss of pineal beta‐adrenergic responsiveness in this species occur endogenously (probably entrained) rather than from gating by acute effects of morning light. Changes in daytime serum aMT consistent with concomitant changes in the pineal can be measured with a sufficiently sensitive radioimmunoassay.

Melatonin is a hormone that is secreted by the pineal gland in the brain. Its secretion during periods of darkness represents a 24-hour rhythm controlled by the circadian clock and is pivotal in the regulation of sleep and wake cycles. Although circadian rhythms are endogenous processes, they can be affected by environmental factors such as light and temperature. In addition to its role in circadian rhythms, melatonin has also been observed to function in the immune system, female menstrual cycles, seasonal behavior, tumor inhibition, and anti-aging processes.[1] Melatonin production increases during the night and decreases in the daytime with exposure to light. Low levels of normal melatonin have been linked to neoplasia, such as the growth of rat hepatomas and human breast cancer xenografts, circadian phase shifts, sleep disturbances, and immunosuppression. [3,4] Furthermore, sleep disturbance, not factoring in melatonin production, can lead to immune suppression and a shift to the predominance in cancer-stimulatory cytokines.[4] To combat cancer, studies have shown that melatonin significantly suppresses cell proliferation and induces apoptosis.[2] There have been no direct links determined between abnormal melatonin production observed in sleep disturbances and cancer, so this review will focus on the experimental evidence depicting the role of melatonin in the prevention of cancer development.
