Plasma ghrelin concentrations in type 1 diabetic patients with autoimmune atrophic gastritis

Abstract: Objective: Type 1 diabetes mellitus patients (DM1) show increased prevalence of pernicious anaemia, the histological substrate of which is type A chronic atrophic gastritis (CAG) in the stomach corpus, the main source of ghrelin. We aimed to compare plasma ghrelin concentrations in DM1 patients with type A CAG (DM1-CAG), DM1 patients without type A CAG and healthy controls and in DM1-CAG group, to ascertain a possible relationship between ghrelin and biochemical markers of gastric mucosa atrophy and/or neuroen… Show more

“…Ghrelin has been reported to be abnormal in other gastrointestinal diseases such as inflammatory bowel diseases (29), coeliac disease (30)(31)(32)(33) and atrophic gastritis (34)(35)(36). Thus, ghrelin seems to be involved in the pathophysiology of gastrointestinal diseases/disorders.…”

“…Whereas ghrelin serum levels were reported to be low, and to be the most sensitive and specific non-invasive marker for gastric body gastritis (30), ghrelin concentration decreased in patients with autoimmune atrophic gastritis and diabetes type 1 (31). In patients with chronic atrophic gastritis acetylated ghrelin levels increased in the gastric body and fundus.…”

“…Patients positive for nuclear antibodies (ANA), thyroperoxidase antibodies (TPOa), tyroglobulin antibodies (TGa), and transglutaminase antibodies or with autoimmune disease-related clinical symptoms were excluded. These patients were recruited from the Department of Endocrinology-Diabetology outpatient clinic of the University Hospital Germans Trias i Pujol (Badalona, Spain) [38,39]. The diagnosis of CAG was based on the presence of low-serum pepsinogen I concentration, hypergastrinemia, pepsinogen I/II ratio under 3, positive PCA, and histologic confirmation as previously described [39].…”

“…These patients were recruited from the Department of Endocrinology-Diabetology outpatient clinic of the University Hospital Germans Trias i Pujol (Badalona, Spain) [38,39]. The diagnosis of CAG was based on the presence of low-serum pepsinogen I concentration, hypergastrinemia, pepsinogen I/II ratio under 3, positive PCA, and histologic confirmation as previously described [39]. Peripheral blood samples were also obtained from 15 DM1 patients (DM1) and 15 age-and sex-matched controls in whom the previously mentioned organ-specific autoantibodies as well as PCA were also negative.…”
