Plasma filtration in the treatment of graves' ophthalmopathy: A randomized study

Abstract: The aim of this study was to perform a randomized study to evaluate the role of plasma filtration in the treatment of severe thyroid-associated ophthalmopathy (TAO). 20 patients were enrolled, and all patients were treated with methylprednisolone IV pulses. 10 randomly chosen patients were also subjected to plasma filtration (twice weekly in Weeks 1, 2, 4, 7, and 10). The procedure proved to be safe. All immunoglobulin classes as well as autoantibodies directed against thyroglobulin, thyroid peroxidase, and TS… Show more

“…6 In search of better outcomes, almost all types of therapy have been investigated, including glucocorticoids (GC), thalidomide, orbital radiotherapy, orbital decompression, somatostatin, intravenous (IV) immunoglobulins, cyclosporine and plasma exchange. [7][8][9][10][11][12][13] Moreover, total thyroidectomy alone can be an advisable choice to treat hyperthyroidism, which also improves GO in Graves' patients. 14 GC, the most common immunosuppressants, have been shown to be a mainstay and well-established therapy for active GO, although it is associated with a variety of side-effects due to the amount of steroids used and the duration of the therapy.…”

“…In a randomized trial of 20 patients with severe ophthalmopathy who received IV glucocorticoids, multiple sessions of apheresis were done in 10 patients. Improvement occurred sooner and was more pronounced in the apheresis group (62). No change in proptosis and eye muscle involvement was noted.…”

“…To date a single randomized trial has been conducted exploring the role of plasmapheresis as an adjunctive treatment in EO . The trial performed by Cap and colleagues in the Czech Republic sought to measure the effect of apheresis on EO via clinical and laboratory parameters and enrolled 20 patients with severe EO who had not experienced improvement in the clinical severity of their disease with the standard treatment of 60 mg of oral prednisone for 2 months.…”
