Introduction: Plasma cell vulvitis (PCV), also known as Zoon's vulvitis, is a rare, chronic inflammatory disease characterized by erythematous plaques with a red, orange hue and increased plasma cells typically affecting the vulva. Case Series: This case series details two clinical presentations of plasma cell mucositis affecting the vagina and cervix, resulting in copious, purulent vaginal discharge. Vaginal and cervical histopathologic findings of a plasmacytic infiltrate confirmed the diagnosis of plasma cell mucositis. Both women were successfully treated with a vaginal estradiol/hydrocortisone/ clindamycin cream and experienced complete resolution of symptoms and physical exam findings. Conclusion: Plasma cell mucositis may be present on the vulva, vagina, and cervix.

Plasma cell balanitis of Zoon (PCBZ) and plasma cell vulvitis (PCV) are characterized as idiopathic, benign, chronic irritant mucositis. The clinical symptoms and signs usually persist or reappear after treatment withdrawal. Therefore, many therapies have been tried and are available. Recently, several reports of PCBZ and PCV treated with calcineurin inhibitors, tacrolimus and pimecrolimus, have been reported in the literature. We present 9 cases of PCBZ treated with tacrolimus 0.1% ointment (Protopic, Toyama, Japan) that showed good therapeutic results within 4 weeks of treatment, and we review the literature of PCBZ and PCV and their response to these topical immunomodulators. Based on the current literature and on the anecdotal experience, we believe that topical calcineurin inhibitors may serve as a therapeutic option in recalcitrant plasma cell balanitis and vulvitis.

Plasma cell vulvitis (PCV) is a rare disease affecting anogenital skin. It is also known by multiple names including Zoon's vulvitis and vulvitis circumscripta plasmacellularis. It is an inflammatory condition that can cause severe local discomfort and psychosocial distress. The diagnosis is challenging and often remains unrecognized. It usually presents as a glistening, red, well-demarcated plaque or patch. The pathophysiology is unknown. Treatment recommendation is mostly based on case reports and case series and this condition is often recalcitrant to topical and systemic treatment.
