Plasma-cell granuloma of the fourth ventricle

Abstract: The case is presented of a 36-year-old woman with a plasma-cell granuloma arising in the fourth ventricle. The mass appeared on computerized tomography as a well circumscribed area of slightly high density that was markedly enhanced with contrast medium. Microscopic examination showed a mixed-cell population with a predominance of plasma cells, and the tumor was characterized immunohistochemically by polyclonal plasma-cell proliferation.

“…Although lesions closely corresponding to Gagel granuloma have not been previously considered in other regions of the body, light, ultrastructural and immunohistological observations in our patients are similar to those described in other cases of plasma-cell granuloma (Kuzela 11 ;Toccanier et al 15;Leonardo and Palestro 12, 1978;Maeda et al ~). Hence, on the basis of the present results, it can be postulated that the socalled Gagel type of granuloma is probably a plasmacell granuloma of the neurohypophysis and hypotha-!amic region.…”

“…Only three patients have been reported so far; lesions in these cases were located in the meninges of the spinal cord. (Eimoto etal.4), tentorium (West et al 16) and fourth ventricle (Maeda et al 13). Here we report on two further patients with plasma-cell granuloma of the hypothalamic region.…”

“…[6][7][8] However, inflammatory pseudotumors of the spleen are extremely rare, with only 67 cases, including our own, ever reported. When the lesion occurs as a primary splenic tumor, lymphoma is usually clinically suspected, and the true diagnosis is usually only revealed after the spleen is examined histologically.…”

“…4 Intracranial plasma-cell granuloma become symptomatic when the adjacent brain is compressed; surgical excision has been the treatment of choice. 5 In some reported patients with subtotal resection or recurrence, irradiation 6,7 or steroid treatment 6 has been used. However, the efficacy is unclear to date.…”
