Plasma Cell Granuloma of the Lung - Resection and Steroid Therapy

Abstract: We report on three resected cases of intrapulmonary plasma cell granuloma. All of them were asymptomatic. One of them showed bilateral multiple lesions and in this case bilateral partial resections of lung in one stage were performed. Under light and electron microscopy the lesions were composed of plasma cells and other kinds of inflammatory cell. The plasma cells were mature and had rich endoplasmic reticulum but no kind of intracytoplasmic inclusion. Steroid therapy was effective for the recurrent lesion af… Show more

“…Because of this finding, measuring serum IgG4 levels may make surgery unnecessary by distinguishing sclerosing pancreatitis from pancreatic cancer. The effectiveness of steroid therapy also has been suggested for IPT-L [20,21], as is shown by case 9 in this study, although it is necessary to diagnose IPT-L correctly without surgery before starting steroid therapy. Given the clinicopathologic similarities between sclerosing pancreatitis and IPT-L, serum IgG4 levels might be elevated in patients with IPT-L and become a diagnostic marker for this disease.…”

“…However, a favorable prognosis for bronchioloalveolar carcinoma (BAC), a histological subclass of lung adenocarcinoma, argues for refining such distinctions (5,6). In addition, metastases of nonlung origin can be difficult to distinguish from lung adenocarcinomas (7,8).…”

“…Reports of radiotherapy being effective 11 and not effective 7,12 have been published, and the role of steroids is uncertain. Thus, if complete resection is not possible or recurrence has developed, radiotherapy and steroids 13 may be beneficial.…”
