Plasma C5 Glucose–to–2H2O Ratio Does Not Provide an Accurate Assessment of Gluconeogenesis During Hyperinsulinemic-Euglycemic Clamps in Either Nondiabetic or Diabetic Humans

Abstract: OBJECTIVE-Measurement of plasma C2 glucose enrichment is cumbersome. Therefore, the plasma C5 glucose-to-2 H 2 O rather than the plasma C5-to-C2 glucose ratio commonly has been used to measure gluconeogenesis and glycogenolysis during hyperinsulinemic-euglycemic clamps. The validity of this approach is unknown. RESEARCH DESIGN AND METHODS-Ten nondiabetic and 10 diabetic subjects ingested2 H 2 O the evening before study. The following morning, insulin was infused at a rate of 0.6 mU ⅐ kg Ϫ1⅐ min Ϫ1 and glucose … Show more

“…Fractional gluconeogenesis measured in this study appears to be 10% higher than in some other studies (1,6,7,13,16) but not all (3,30), as shown in Table 3. Methodological differences may explain the variations of fractional gluconeogenesis, such as the administration procedure for oral doses of D 2 O and analytical methods (average method vs. HMT derivative of glucose, and C2 vs. plasma or urine body water 2 H enrichment).…”

“…As for the effects of ingesting an HP meal, one study pointed out that the ingestion of 50 g meat proteins (without carbohydrates) led to a slight but steady decrease in EGP in subjects with type 2 diabetes (33). EGP is upregulated in diabetic patients, but this impairment is especially marked when insulin secretion is stimulated (3436). Thus, in the case of a carbohydrate-free meal, and despite a slight stimulation of insulin secretion, it is likely that the EGP response will be similar in healthy and diabetic patients.…”

“…Deuterated water ( 2 H 2 O) reveals the fraction of blood glucose derived from glucose 6-phosphate (G6P) via glucose 6-phosphatase (Basu et al, 2008;Nunes and Jones, 2009). In addition, the sources of G6P can be resolved into gluconeogenic and glycogenolytic contributions by measuring the ratio of enrichment in positions 5 and 2 (H5/H2) of blood glucose (Landau et al, 1996) (Fig.…”
