The very rare occurrence of an ADH-producing small cell carcinoma of the lung in a 52 year old male patient with cranial diabetes insipidus since childhood is described. In this case diabetes insipidus disappeared concomitantly with development of lung cancer and re-appeared with shrinkage of the lung tumour by radiation therapy. Further progressive expansion of the primary and metastatic tumours induced the syndrome of inappropriate ADH secretion once again (SIADH). This deterioration in the clinical course was reflected in the plasma levels of ADH and neurophysins. The existence of vasopressin in the tumour tissue was also demonstrated by means of an immunohistochemical staining technique combined with anti-vasopressin serum.

Hyponatremia is an important and common electrolyte disorder in tumor patients and one that has been reported in association with a number of different primary diagnoses. The correct diagnosis of the pathophysiological basis for each patient is important because it significantly alters the treatment approach. In this article, we review the epidemiology and presentation of patients with hyponatremia, the pathophysiologic groups for the disorder with respect to sodium and water balance and the diagnostic measures for determining the correct pathophysiologic groups. We then present the various treatment options based on the pathophysiologic groups including a mathematical approach to the use of hypertonic saline in management. In cancer patients, hyponatremia is a serious co-morbidity that requires particular attention as its treatment varies by pathophysiologic groups, and its consequences can have a deleterious effect on the patient's health.

Unter paraneoplastischen Syndromen (PNS) versteht man Fernwir-kungen von Tumoren im Organismus, die nicht durch Invasion, Obstruktion oder durch die Tumormasse selbst verursacht sind. Am besten untersucht sind PNS durch vom Tumor gebildete Hormone, insbesondere durch Pro-ACTH und dessen funktionell wirksame Untergruppen, durch ADH, Gonadotropine, HPL, Prolactin u.a. Häufig kommt es zu charakteristischen Krankheitsbildern, die genauer beschrieben werden. Daneben werden häufig verschiedenste PNS beobachtet, die weniger gut definiert sind, insbesondere des Nervensystems, der Hämopoese, der Nieren, der Haut und des Magen-Darm-Traktes sowie in seltenen Fallen verschiedene charakte-ristische Syndrome, die aber sehr typisch sind für bestimmte Tumoren.
