Plantarflexion Opening Wedge Medial Cuneiform Osteotomy for Correction of Fixed Forefoot Varus Associated with Flatfoot Deformity

Abstract: Opening wedge medial cuneiform osteotomy is an important adjunctive procedure to correct the forefoot varus component of a flatfoot deformity. Advantages of this technique in comparison to first tarsometatarsal arthrodesis include predictable union, preservation of first ray mobility, and the ability to easily vary the amount of correction. Because of the variety of hindfoot procedures done in these patients, the degree of hindfoot correction contributed by the cuneiform osteotomy alone could not be determined… Show more

“…In our study, 3 cases of subluxation developed at the calcaneal-cuboidal joint in the LCL group, although all the feet were free of symptoms at the last follow-up visit. However, complications after calcaneal-cuboid-cuneiform osteotomy include painful hardware, nerve injury, wound infection, and delayed union (6,21). In our study, 1 case of superficial wound infection was encountered in each group was treated with antibiotics.…”

“…14 MCOWO has the advantage of correcting the forefoot supination deformity, therefore avoiding arthrodesis. [15][16][17] It is generally believed that the two biggest advantages of MCOWO were a higher union rate than that of midfoot or ankle joint arthrodesis and a varied degree of correction with a preservation of first ray motion.…”

“…To achieve a lowering of the medial ray, the authors prefer the Cotton osteotomy (plantarflexion osteotomy of the medial cuneiform), which is performed through a dorsal approach. 17 After vertical osteotomy of the medial cuneiform, the bone is spread and filled with an autologous or allogenic allograft. At times there is need to secure the construct with a small plate, which is placed dorsally.…”
