Plantar Purpura as the Initial Presentation of Viridians Streptococcal Shock Syndrome Secondary toStreptococcus gordoniiBacteremia

Abstract: Viridians streptococcal shock syndrome is a subtype of toxic shock syndrome. Frequently, the diagnosis is missed initially because the clinical features are nonspecific. However, it is a rapidly progressive disease, manifested by hypotension, rash, palmar desquamation, and acute respiratory distress syndrome within a short period. The disease course is generally fulminant and rarely presents initially as a purpura over the plantar region. We present a case of a 54-year-old female hospital worker diagnosed with… Show more

“…S gordonii is a gram-positive, nonmotile, coccal, facultative anaerobic genus of the viridans group Streptococcus. [ 6 ] The Streptococcus viridans group is a heterogeneous group of organisms with controversial taxonomy [ 7 ] and can be further subclassified into 6 major groups: the Streptococcus anginosus group, Streptococcus mitis group, Streptococcus sanguinis group, Streptococcus bovis group, Streptococcus salivarius group, and Streptococcus mutans group. The S sanguis group includes S sanguis , Streptococcus parahaemolyticus , and S gordonii [ 8 ] (Fig.…”

“…S. gordonii is a Gram-positive, non-motile, coccus belonging to the viridians streptococcal group [2]. Generally present in the mouth and upper airway, this facultative anaerobe is capable of spreading to extraoral sites and causing infective endocarditis, septic arthritis, and toxic shock-like syndrome [3, 4]. Furthermore, some reports have described empyema caused by periodontal disease [5].…”

“…S. gordonii is commonly found in the mouth and is capable of spreading to extraoral sites, causing systemic infection [ 1 ]. S. gordonii , along with S. sanguinis and S. oralis, is commonly found in blood cultures of patients with infective endocarditis [ 2 ]. It is capable of colonizing platelet-fibrin thrombi on abnormal heart valves or the endocardium.…”
