Objective: To compare the diagnostic and therapeutic effect of endoscopically assisted arthrocentesis with conventional arthrocentesis and arthroscopy.Material and Methods: 372 TMK examinations and treatments were performed from January 2019 to April 2021 (27 months). We monitored the pain level on the VAS scale, the extent of interincisal distance, and the value of NMR scans before and after treatment.Results: By comparing the results of clinical examination, graphical imaging and statistic evaluation of conventional arthrocentesis under local anesthesia, conventional arthroscopy under general anesthesia, and innovative endoscopic-assisted single cannula three-way arthrocentesis under local anesthesia, it can be seen that the postoperative interincisional distance and grade values for pain after surgery are better for surgical procedures under local anesthesia with the innovative endoscopicassisted single cannula three-way technique. Particularly significant are the improved values for the larger range of interincisional distance. Conclusion:By assessing the intraarticular structures based on NMR examinations and the Wilkes classification scheme, we found that all minimally invasive procedures resulted in improved pathologies. Based on clinical experience, we can conclude that the innovative endoscopically assisted single cannula three-way arthrocentesis under local Anaesthesia is superior to conventional arthroscopy under general Anaesthesia and conventional arthrocentesis under local Anaesthesia.

OBJECTIVE: Analyse, compare and evaluate the effects of the treatment arthrocentesis with a control group of non-steroid drugs treated patients. MATERIAL AND METHODS: 1752 outpatients (1293 females, 459 males) were examined within the years 2013-2017. We evaluated the following criteria: gender, age, visual analog scale for pain (VAS), inter-incisal distance and reducing intake of orally administered analgesics. RESULTS: The pain level of the group of patients with arthrocentesis had stabilized at mark 2.5 on the pain scale, unlike in the control group after treatment with non-steroidal anti-infl ammatory drugs, pain had stabilized at mark 9 on the pain scale. The inter incisal distance amongst the fi rst group was 37 mm and in the control group only 27 mm, after completed treatment. The amount of applied analgesics in the group with arthrocentesis decreased to 100 mg daily, while in the control group, the dose was adjusted to 700 mg daily. CONCLUSION: In the observed study, arthrocentesis was effective in reducing pain, amount of analgesics per day and improving the mobility of temporomandibular joint (TMJ). The results of this study we use in the ongoing project, that focuses on progressive and innovative methodology of endoscopically assisted arthrocentesis.

In recent decades, there have been important developments in temporomandibular joint (TMJ) imaging. New techniques complement preexisting methods, and each modality is indicated for specific conditions in accordance with the clinical diagnosis. The aim of this review is to give an overview of conventional and new techniques in TMJ imaging. The literature review includes studies published between 1976 and 2009 that appear in the Medline database. Magnetic resonance imaging remains the ''gold standard'' modality for TMJ analysis. It allows structures to be visualized without radiation whether or not they are mineralized; however, it is costly and complex. Ultrasonography, an alternative method with increasing importance in TMJ analysis, is a simple, noninvasive, and low-cost technique that allows for the visualization of the position of the disk; however, it does not detect condylar abnormalities. Reconstructions in three-dimensions can be obtained with computed tomography, magnetic resonance imaging, and ultrasonography and can be used to obtain rapid prototyping biomodels. Health professionals performing TMJ imaging exams should consider clinical history and findings, exam cost, radiation exposure, results of previous exams, and whether the current result will influence diagnosis and treatment planning.
