Objectives. To examine the association between placenta previa with maternal race and its variations by country of origin among Asian women. Study design. Retrospective cohort study. Methods. We analyzed data from a population-based retrospective cohort study of 16,751,627 pregnancies in the US. The data were derived from the national linked birth/infant mortality database for the period 1995Á2000. Multiple logistic regressions were used to describe the relationship between placenta previa and race as well as country of origin among Asian women. Results. About 3.3 per 1,000 pregnancies were complicated with placenta previa among white women, while the corresponding figures for black women and women of other races were 3.0 and 4.5 per 1,000 pregnancies, respectively. The excess risk remained substantial and significant after adjustment for confounders for women of other races compared to white women. The frequencies of placenta previa among Chinese, Japanese, Filipino, Asian Indian, Korean, Vietnamese and other Asian or Pacific Islander were 5.6, 5.1, 7.6, 4.5, 5.9, 4.4 and 4.4 per 1,000 pregnancies, respectively. The adjusted odds ratios ranged from 1.39 to 2.15 among Asian women by country of origin, with the lowest for Japanese and Vietnamese and the highest for Filipino women in our study. Conclusion. Asian women have excess risk of placenta previa compared with white women. Major variation exists in placenta previa risk among Asian women, with the lowest risk in Japanese and Vietnamese women and the highest risk in Filipino women.

Maintenance Tocolytics for Preterm Symptomatic Placenta Previa: A Review

The purpose of the review article is to determine if prolonged (≥48 hour) tocolytics with symptomatic preterm placenta previa improves perinatal outcome. OVID MEDLINE and Cochrane Databases were searched from January 1950 to January 2009. Odds ratio (OR) and 95% confidence intervals (CI) were calculated. We identified two retrospective studies ( N = 217) and one randomized clinical trial (RCT; N = 60), and they were analyzed separately. Results of the RCT indicated that pregnancy is prolonged for more than 7 days with continued tocolytics (OR 3.10, 95% CI 1.38 to 6.96) but combined results of two retrospective studies did not confirm the prolongation (OR 1.19, 95% CI 0.63 to 2.28). The RCT was inadequately compliant with Consolidated Standards of Reporting Trials statement. While awaiting an appropriately designed RCT to determine the duration of tocolytics with placenta previa and preterm labor, it should be limited to 48 hours.

Tocolysis can be used to arrest contractions in selected patients with placenta previa if the maternal condition is stable. Over a 5-year period, 41 patients with symptomatic placenta previa were treated, of whom 18 were given magnesium sulfate therapy for tocolysis. The mean prolongation of gestation was 18.5 days, and tocolysis was successful in 17 of 18 cases. Since betamimetic drugs used for tocolysis may mask or blunt maternal cardiovascular responses to volume depletion, magnesium sulfate is a better choice to inhibit contractions in patients with symptomatic placenta previa whose bleeding is mild or moderate.
