Placebo-controlled comparison of the efficacy and tolerability of once-daily moxonidine and enalapril in mild-to-moderate essential hypertension

Abstract: Moxonidine is an effective and well-tolerated antihypertensive, at least as good as other established forms of antihypertensive medication. The trough:peak ratio of 0.7 indicates that the drug will be effective administered once a day.

“…5 Moxonidine effectively reduces blood pressure; however, side effects such as dizziness and dry mouth are much less than with the older centrally acting antihypertensives, ie, clonidine. 10 Indeed, these side effects seem to be due to activation of central ␣ 2 -receptors. Moxonidine, however, stimulates the imidazoline-1 receptors in the ventrolateral area of the medulla oblongata, which, at least in animals, leads to a decreased sympathetic tone in resistance vessels, the heart, and the kidney.…”

“…28 Finally, moxonidine produced no significant effect on heart rate, which is also consistent with the results of earlier trials. 27,29,30 This may be related to a lack of effect on the vagal control of heart rate, which is greater than the sympathetic control at rest, as confirmed by an absence of change in cardiac BRS. Also, any moxonidinemediated decrease in heart rate due to its sympatholytic effect may be opposed by a reflex increase in heart rate through vagal inhibition resulting from the decrease in arterial pressure.…”
