Placebo control study of acute smokeless tobacco abstinence in young adult men.

Abstract: Tobacco use can lead to dependence, as indicated by withdrawal symptomatology during abstinence. In smokers, nicotine-free cigarettes suppress tobacco withdrawal, suggesting that non-nicotine stimuli may be relevant to tobacco dependence. This study examined non-nicotine factors in smokeless tobacco (SLT) withdrawal. SLT users used their own brand of SLT, nicotine-free SLT, or no SLT hourly in 3 approximately 4.5-hr sessions. Participant-rated measures of craving and desire to use SLT were elevated in the abst… Show more

“…With respect to particular products, in Study 1 we observed that General snus, but not Stonewall, approximated Own Brand nicotine exposure (consistent with Hatsukami et al, 2004), though Stonewall and General snus both suppressed withdrawal acutely (as did the nontobacco placebo; Gire & Eissenberg, 2000). Study 1 results with respect to nicotine exposure and withdrawal suppression were also consistent with data from Study 2.…”

“…Two of the measures (taken from Gire & Eissenberg, 2000) were presented as visual analogue scales (VAS) that consisted of an item above a horizontal line that had anchors on the left (“not at all”) and right (“extremely”). Subjects moved a mouse controlled cursor and clicked to produce a vertical mark on the horizontal line.…”

“…The de-nicotinized cigarette condition allowed another demonstration of placebo-induced withdrawal suppression in tobacco users (e.g., Baldinger et al, 1995;Butschky, Bailey, Henningfield, & Pickworth, 1995;Gire & Eissenberg, 2000). In this case, de-nicotinized cigarettes were as effective as normally marketed cigarettes in suppressing acute withdrawal symptomatology on some measures (e.g., the QSU Factor 1).…”

“…This placebo-induced withdrawal suppression is thought to arise from smoking-related stimuli that gain their withdrawal suppressing capability via classical conditioning, wherein tobaccorelated stimuli (e.g., taste, smell of smoke, sight of lit cigarette, etc.) become associated with nicotine delivery (e.g., Gire & Eissenberg, 2000;Rose, Behm, & Levin 1993). Other factors, such as non-nicotine mediated MAO-B inhibition (Fowler et al, 1998) may also contribute to the effect.…”

“…This appears to be based on relatively high rates of self-reported dependence among users, 34,35 the fact that nicotine intake appears to be similar in amount to that from cigarettes, 36 that users can obtain relatively high scores on a smokeless tobacco version of the FTND, 37 and reports of cravings and withdrawal symptoms during abstinence. 38 Using questions generated from the FTND, variations in levels of dependence, beliefs, knowledge and attitudes were observed between women chewing tobacco in paan and those chewing paan alone. 13 It may be noted however, that assessing level of dependence based on frequency of usage can be problematic.…”
