Pisat’s Visual Vasopressor Injection Needle: A New Device for Increasing Patient Safety in Laparoscopic Myomectomy

Abstract: Using a VVIN during a laparoscopic myomectomy enables the surgeon to detect an inadvertent vascular puncture very early, even in a small calibre blood vessel, and with much more sensitivity than a regular needle. This increases patient safety during the intra-myometrial injection of a vasoconstrictive agent during myomectomy and reduces the incidence of catastrophic complications.

“…This increases patient's safety. 7 Uterine fibroids are rich in blood supply and the possibility of large dose of vasopressin getting absorbed into the blood vessels cannot be excluded. Coronary vasoconstriction, increased vagal tone, and sympathetic tone cause bradycardia, arrhythmias, decreased cardiac output, and, in severe cases, myocardial ischemic and death.…”

“…By adding a small, transparent area at the proximal end of the instrument, a blood-stained aspirate can be immediately seen in case the needle has punctured a vessel. 4 The needle can then be repositioned at another site over the fibroid. This significantly reduces the possibility of injecting the vasopressor solution in the vascular channels over the fibroid.…”

“…Injection vasopressin (dilution 20 unit in 200 m NS) was injected with the help of visual vasopressor injection needle[ 1 ] which resulted in blanching [ Figure 1b ]. Fibroid was excised and two flaps of round ligament seen after enucleation of fibroid [ Figure 1c ].…”
