“…This holds true for conventionally fractionated postoperative radiotherapy, as well as for hyperfractionated radiation schedules [3,12]. Other radiation modalities, such as the use of neutrons [8,16], of pions [7] or heavy ions [4] did not live up to expectations. “…In cases believed to have transformed from lower grade gliomas or when the region of enhancement was ill defined a more generous high dose volume usually incorporating the low density region was used. Eighty of these received Megavoltage x-ray therapy, but 34 cases were offered and agreed to receive pion radiation therapy at TRIUMF during the first five years of the dose escalation program when the pion-beam was avail- [10]. Pion therapy has provided similar survival results to conventional therapy, although the phase II1 trial is ongoing.…”

“…The subset of patients who received pion therapy behaved in a similar fashion to all other patients in this analysis. This does not include the whole experience with the pion treated patients at TRIUMF which has been conducted since 1982 and separately reported [10]. Other clinical approaches that are being explored include the use of hypoxic chemosensitization, localized accelerated radiotherapy (60 Gy in 30 fractions over three weeks) and the use of radioactive implantation for recurrent malignant glioma.…”

“…Currently this treat ment technique is investigated in several centers [7,45]. In some centers the application of pion therapy of malignant astro cytoma is under investigation [23,25], Using a 3-dimensional spot technique the accuracy of the beam alignment was repro ducible within 2 mm. From a base line dose of 40 Gy photons whole brain and 17.5 Gy pion boost, doses were escalated to 33 Gy pions localized to the primary tumor and the median survival improved from 198 to 436 days.…”
