Piloting a partially self-financed mode of human immunodeficiency virus pre-exposure prophylaxis delivery for men who have sex with men in Hong Kong

Abstract: Pre-exposure prophylaxis (PrEP) with tenofovir disoproxil fumarate (TDF) 300 mg/ emtricitabine (FTC) 200 mg is a proven strategy for preventing human immunodeficiency virus (HIV) transmission in men who have sex with men (MSM). This study aimed to test the feasibility and acceptability of PrEP delivered at a pilot clinic for MSM in Hong Kong, where PrEP service is currently unavailable. Methods: Partially self-financed PrEP was provided to HIV-negative adult MSM with high behavioural risk of HIV transmission a… Show more

“…Eligible participants were MSM, aged 18 years or above, who had had CLAI with men in the preceding 6 months, inclined to have CLAI in the coming 6 months, were HIV-negative, not hepatitis B carriers, had a creatinine clearance of at least 60 ml/min and occurrence of at least one behavioural risk in the past 6 months, including chemsex engagement, STI diagnosis, had multiple sex partners and had people living with HIV (PLHIV) as sex partners regardless of their viral load status [11]. Participants were ineligible if they were unable to communicate in either Chinese or English, were not normally residing in Hong Kong, or had any form of mental illnesses.…”

“…In Hong Kong, there have not been any government-funded PrEP programmes and only patented TDF/FTC was available through prescriptions, the cost of which was well beyond the local MSM's affordability [11]. Despite the use of identical antiretrovirals, on-demand and daily PrEP each requires very different forms of life adjustment.…”

“…The first project offered partially self-financed TDF/FTC for daily PrEP ( n = 71); the logistical procedure of which has been described elsewhere. 26 The second project was a cross-over study ( n = 119) involving the prescription of free daily followed by on-demand PrEP and vice versa each for a period of 16 weeks. 27 During the on-demand phase, participants took two tablets of TDF/FTC 2–24 h before sex followed by one tablet each at 24 and 48 h afterwards, referenced from the regimen for the IPERGAY study.…”

“…The ever-testing rate among MSM ranged from 52% in Brazil [ 5 ], 70% in China [ 6 ], and 85% in the United States [ 7 ]. In Hong Kong, where MSM accounted for a majority of the newly diagnosed HIV cases [ 8 ] and pre-exposure prophylaxis (PrEP) is only available from clinical studies but not public health care services [ 9 , 10 ], only 64% had ever tested for HIV and half (50%) of the population last tested within a year [ 11 ]. Various reasons were cited as barriers to HIV testing in the community, such as low perceived risk and poverty [ 11 , 12 ].…”
