Background During the COVID-19 pandemic, the University of Leipzig completely switched to online teaching. Thus, we developed a practice-oriented digital substitute for a two-week mandatory general practice (GP) clerkship. Main components were processing of clinical cases and additional GP topics, visual diagnoses, information and examination videos, and regular remote exchanges with associated GP teachers. We took the chance to comprehensively evaluate the new teaching formats (acceptance, use, working enjoyment, learning gain, practical relevance, insights into general practice) and to compare evaluations with two previous semesters to gain insights for future blended learning concepts. Methods Cross-sectional post-hoc online evaluation among fourth year (of six) medical students participating in the digital mandatory 2-week GP clerkship during summer semester 2020; additional cohort comparison with two previous semesters (face-to-face clerkship). Results Out of 192 participants in the digital clerkship, 99 completed our questionnaire (response rate = 51.6%). Results were compared with 277 previous evaluations (face-to-face semesters). Most participants reported having enjoyed the online-based clerkship (87.9%), having learned a lot (89.9%), having gained insights into general practice (76.8%), and perceived high practical relevance (90.9%). Implementing the new teaching formats into future face-to-face clerkships was welcomed by 65.6%. Clinical cases, visual diagnoses, examination videos and communication with GP teachers were rated best regarding working enjoyment, learning gain, practical relevance and insights into a GP’s work. Cohort comparison revealed somewhat better evaluations regarding knowledge transfer for the digital clerkship while imparting of skills and attitudes was reportedly worse. Conclusions Students welcomed the digital content and perceived relevant learning gain. Our results may help to develop future blended learning concepts. Clinical cases, examination videos and visual diagnoses appear to be particularly suitable as useful online complements which could enrich face-to-face teaching. As students especially valued the exclusive time for exchanges with their preceptor, this should be facilitated in face-to-face clerkships.

Ensino híbrido na formação em saúde: uma revisão sistemática

Introducción: Ante la necesidad de reestructurar las instituciones de educación en salud a causa de la pandemia por COVID-19, se ha señalado a la educación híbrida como una alternativa para reorganizar las actividades educativas. El objetivo de este estudio es describir el desarrollo de la educación híbrida en la formación de los profesionales en el área de la salud. Materiales y métodos: Revisión sistemática de la literatura basada en las recomendaciones de los elementos de informes preferidos para revisiones sistemáticas y metaanálisis (PRISMA). La búsqueda se realizó por pares entre julio y septiembre de 2020 en cuatro bases de datos electrónicas. Los descriptores utilizados fueron “educación a distancia”, “educación superior”, “aprendizaje”, “salud”, “enfermería”, “medicina”, “odontología”, “fisioterapia” y “educación híbrida”. A su vez, los artículos seleccionados se clasificaron según su nivel de evidencia. Resultados: Se seleccionaron 49 artículos, entre los que se incluyeron estudios cuantitativos, cualitativos y de métodos mixtos. Se identificaron experiencias en el desarrollo de la educación híbrida en diferentes cursos de formación en el área de la salud. Se observó que la educación híbrida se implementó de acuerdo con el modelo de rotación, el modelo a la carta y el modelo Flex. Discusión: La enseñanza híbrida ha venido ganando cada vez más importancia en el escenario académico de la educación en salud. Se observó que, a partir de esta, el estudiante logra destacarse en su aprendizaje, ya que se convierte en el principal gestor de este proceso al aprender de forma activa mediante diversas herramientas educativas guiadas por el docente. Conclusiones: El éxito de la educación híbrida se puede relacionar con su carácter innovador, flexible, con buena relación costo-beneficio y la habilidad de convertir a los estudiantes en protagonistas de su proceso de enseñanza-aprendizaje, lo que influye en su propio rendimiento académico. Zusammenfassung Hintergrund Mit der neuen Approbationsordnung soll das wissenschaftliche Arbeiten stärker in das Medizinstudium integriert werden. Ziel der Arbeit/Fragestellung Daher sollte im Fachgebiet Allgemeinmedizin überprüft werden, wie die Integration einer wissenschaftlichen Arbeit mit Patient*innen in das bestehende Curriculum von den Studierenden umgesetzt werden kann und wie die Lehrärzt*innen diese Erweiterung der Lehre annehmen. Material und Methoden Dazu sollten die Medizinstudierenden Patienten*innen mit einer Allopurinolbehandlung in Hausarztpraxen untersuchen. Hierfür sollten neben einer persönlichen Befragung der Patient*innen auch Daten aus den Patient*innenakten entnommen werden. Die Eintragungen in den Untersuchungsbögen wurden anschließend anhand der Patienten*innenakten auf die korrekte Dateneingabe überprüft. Zudem wurden die Lehrärzt*innen anhand eines Fragebogens zur dem Lehrprojekt befragt. Ergebnisse An dem Lehrprojekt nahmen 35 Studierende in 29 Lehrpraxen teil. In sieben Lehrpraxen (25 %) wurden wie geplant Patient*innen mit einer Gichtbehandlung untersucht. Dichotome Items und Fragen zu Personendaten wurden durchweg korrekt von den Studierenden dokumentiert. Fehler bei der Dokumentation konnten bei komplexeren Items, wie z. B. bei der Abfrage der Vormedikation oder den Vorerkrankungen, beobachtet werden. Die Lehrärzt*innen gaben Zeit- und Personalprobleme und Schwierigkeiten bei der Patient*innenrekrutierung an. Schlussfolgerungen Für eine erfolgreiche Integration einer wissenschaftlichen Arbeit in eine bestehende Lehrveranstaltung sollten sich alle beteiligten Lehrkräfte mit den wissenschaftlichen Mitarbeiter*innen möglichst frühzeitig zusammensetzen und die zukünftige Durchführung der wissenschaftlichen Arbeiten gemeinsam planen.
