Introduction Comprehensive Behavioral Intervention for Tics (CBIT) has been shown to be efficacious for Chronic Tic Disorders (CTDs), but utilization is limited by a lack of treatment providers and perceived financial and time burden of commuting to treatment. A promising alternative to in-person delivery is Voice over Internet Protocol (VoIP), allowing for remote, real-time treatment delivery to patients’ homes. However, little is known about the effectiveness of VoIP for CTDs. Therefore, the present study examined the preliminary efficacy, feasibility, and acceptability of VoIP-delivered CBIT (CBIT-VoIP). Methods Twenty youth (8–17) with CTDs participated in a randomized, waitlist-controlled pilot trial of CBIT-VoIP. The main outcome was pre- to post-treatment change in clinician-rated tic severity (Yale Global Tic Severity Scale). The secondary outcome was clinical responder rate (Clinical Global Impressions – Improvement Scale), assessed using ratings of ‘very much improved’ or ‘much improved’ indicating positive treatment response. Results Intention-to-treat analyses with the last observation carried forward were performed. At post-treatment (10-weeks), significantly greater reductions in clinician-rated, [F (1, 18) = 3.05, p < 0.05, partial η2 = 0.15], and parent-reported tic severity, [F (1, 18) = 6.37, p < 0.05, partial η2 = .26] were found in CBIT-VoIP relative to waitlist. One-third (N = 4) of those in CBIT-VoIP were considered treatment responders. Treatment satisfaction and therapeutic alliance were high. Discussion: CBIT can be delivered via VoIP with high patient satisfaction, using accessible, low-cost equipment. CBIT-VoIP was generally feasible to implement, with some audio and visual challenges. Modifications to enhance treatment delivery are suggested.

Background In response to COVID-19, there has been increasing momentum in telehealth development and delivery. To assess the anticipated exponential growth in telehealth, it is important to accurately capture how telehealth has been used in specific mental health fields prior to the pandemic. Objective This systematic review aimed to highlight how telehealth has been used with clinical samples in the neurodevelopmental field, including patients with neurodevelopmental disorders (NDDs), their families, and health care professionals. To identify which technologies show the greatest potential for implementation into health services, we evaluated technologies for effectiveness, economic impact, and readiness for clinical adoption. Methods A systematic search of literature was undertaken in April 2018 and updated until December 2019, by using the Medline, Web of Science, Scopus, CINAHL Plus, EMBASE, and PsycInfo databases. Extracted data included the type of technology, how the technology was used (ie, assessment, treatment, and monitoring), participant characteristics, reported outcomes and authors’ views on clinical effectiveness, user impact (ie, feasibility and acceptability), economic impact, and readiness for clinic adoption. A quality review of the research was performed in accordance with the Oxford Centre for Evidence-Based Medicine Levels of Evidence. Results A total of 42 studies met the inclusion criteria. These studies included participants and family members with autism spectrum disorders (21/42, 50%), attention deficit hyperactivity disorders (8/42, 19%), attention deficit hyperactivity or autism spectrum disorders (3/42, 7%), communication disorders (7/42, 17%), and tic disorders (2/42, 5%). The focus of most studies (33/42, 79%) was on treatment, rather than assessment (4/42, 10%) or monitoring (5/42, 12%). Telehealth services demonstrated promise for being clinically effective, predominantly in relation to diagnosing and monitoring NDDs. In terms of NDD treatment, telehealth services were usually equivalent to control groups. There was some evidence of positive user and economic impacts, including increased service delivery efficiency (eg, increased treatment availability and decreased waiting times). However, these factors were not widely recorded across the studies. Telehealth was demonstrated to be cost-effective in the few studies that considered cost-effectiveness. Study quality varied, as many studies had small sample sizes and inadequate control groups. Of the 42 studies, only 11 (26%) were randomized controlled trials, 12 (29%) were case studies or case series, 6 (14%) were qualitative studies, and 5 (12%) were noncomparative trials. Conclusions Telehealth has the potential to increase treatment availability, decrease diagnosis waiting times, and aid in NDD monitoring. In order to identify which technologies may show greatest clinical and cost-effectiveness for implementation into health services post COVID-19 the review evaluates the technologies for effectiveness, cost efficiencies, and readiness for clinical adoption. The quality of the evidence and gaps in the literature are also considered to guide future work. METHODS A systematic search of the literature was undertaken (April 2018 - updated until December 2019) using the following databases: Medline, Web of Science, Scopus, CINAHL Plus, Embase, and PsycInfo. A quality review of the research is also reported. RESULTS A total of 42 studies met the inclusion criteria. These included participants and/or their families with autism spectrum disorders (ASD; n=22), attention deficit hyperactivity disorders (ADHD; n=8), ADHD and/or ASD (n=3), communication disorders (n=7), and tic disorders (n=2). The majority of studies were with children/young people and/or their caregivers. The focus of most studies was treatment (n=33), rather than assessment (n=4) or monitoring (n=5). In terms of clinical effectiveness, telehealth services were usually equivalent to controls (e.g. face-to-face services or services without therapist input), costs were generally not reported but when they were telehealth was seen as being cost-effective. The quality of the studies varied, with many having small sample sizes and inadequate control groups. CONCLUSIONS The use of telehealth has the potential to increase the availability of treatment, decrease waiting times for diagnosis, and aid in the monitoring of NDD, post COVID-19. Further research, with more robust and adequately powered study designs as well as consideration of cost-effectiveness and efficiency savings, is needed. The review highlights the extent of the use of telehealth technologies prior to COVID-19 and the movement to invest in remote access to treatments.
