Pilonidal Disease

Abstract: Pilonidal disease presents many therapeutic challenges to surgeons throughout the world. Its varied clinical presentations necessitate a wide range of treatments, thus underscoring the need to tailor the treatment to the patient and the severity of disease. Recent studies confirm the efficacy of smaller, more conservative operations for appropriate indications. When flap closures are performed, every attempt should be directed to placing sutures off (lateral) to the midline gluteal cleft. Meticulous attention … Show more

“…7 Management is variable and guided by the clinical presentation and extensiveness of the disease. 14,[16][17][18] Many procedures right from conservative management to various surgical procedures including shaving, incision and drainage, phenol application, cryosurgery, excision with primary closure, excision and open packing, excision with marsupialization, and recently, flap surgeries. None of them is considered as perfect as far recurrence of diseases is concern.…”

“…This is a common source of morbidity and loss of work productivity in healthy young adults (3) . A large number of surgical techniques (with varying complexity) have been described in the literature for the treatment of this disease; many of which are unfamiliar to general surgeons.…”

“…If given the opportunity to drain spontaneously, this may act as a portal of further invasion and eventually formation of a foreign body granuloma. Infection may result in abscess formation (3) . Microscopically, the sinus where the hair enters is lined with stratified squamous epithelium with slight cornification.…”

“…According to a study, about 85% patients require surgery ultimately. (9) Many conservative methods are used and are more acceptable to patients in spite of knowing about the possibility of a recurrence. Injections of Phenol or Fibrin glue after debridements of the track are widely used methods.…”

“…Recurrence rates after primary closure may be as high as 38%. 9 Flap procedures are suitable for complex or recurrent pilonidal disease, but they require expertise. Flap procedures and excisions by assymetrical incisions take care of correction of the anatomically deep intergluteal cleft, which is the major factor in preventing recurrence.…”
