Background: Many complex factors contribute to suicide in older adults. The suicidal ideation that precedes suicide is an especially direct predictor of suicide. This study aimed to identify the effects between variables affecting suicidal ideation among older adults using the International Classification of Functioning, Disability and Health (ICF) model and understand the causal relationships to systematize complex factors. Methods: This study used data from 9920 community-dwelling older adults who completed a national survey in 2020 to classify predictors of suicidal ideation (e.g., depression, subjective health status, sociodemographic factors, health factors, social support, instrumental activities of daily living (IADL), and social participation) by using the ICF model. To determine the causal relationship between variables, this study examined significance based on the critical ratio (C.R.) and squared multiple correlation (SMC) by using a path model. Results: Gender, education level, economic level, age, IADL, relationship satisfaction with a child, depression, and the number of chronic diseases significantly affected suicidal ideation, while age, employment status, participation in social groups, formal and informal support, satisfaction with a friend/neighbor relationship, and subjective health status did not significantly influence it. Moreover, depression mediated the relationship between each of these variables and suicidal ideation. Conclusions: It was found that depression was the most direct and mediating factor in suicidal ideation among many factors affecting the suicidal ideation of community-dwelling older adults. Additional studies should be conducted to develop community-level strategies based on these factors and understand causal relationships.

Purpose: To assess the prevalence of self-neglect and associated factors among disability elderly(age≥65 years) in China.Patients and methods: Patients aged 65 years or above were enrolled from two communities in Shenyang,China. A convenience sample of 230 disability elderly responded to the questionnaire survey. The data were collected by a series of questionnaires consisting of socio-economic and demographic characteristics, Barthel Index(BI), Elder Self-Neglect Assessment(ESNA), Geriatric Depression Scale-Short Form(GDS-SF), Multidimensional Scale of Perceived Social Support(MSPSS), and Mini-Mental State Examination(MMSE). Kruskal-Wallis test or Mann-Whitney U test was used to examine the differences of self-neglect among different demographic characteristics as appropriate.Multiple linear regression model was performed to identify which factors were independent predictors of self-neglect.Results: An overall self-neglect rate of 86.0% was registered. The study reveals that disability elderly's self-neglect correlates positively with the degree of disability and depressive symptoms. On the contrary, monthly income and perceived social support were negatively associated with self-neglect.Conclusion: In this study, the prevalence of self-neglect is high among disability elderly. Lower monthly income, decreased physical functioning, depressive symptoms, and poor perceived social support were contributing factors for self-neglect among disability elderly.

Associations between Suicidal Ideation and Relatives’ Physical and Mental Health among Community Residents: Differences between Family Members and Lineal Consanguinity

(1) Background: Despite the verified relationship between relatives’ characteristics and individual suicidal ideation, few studies have discussed the role of family members and lineal consanguinity independently according to whether they live together with the individuals or not. (2) Methods: The data in this study were collected in November 2019 and identified rural adults over 18 years old in Shandong as the survey objects, with a total of 879 valid cases included in this survey. Logistic regression analysis was employed to examine the risk factors affecting adults’ suicidal ideation and differentiate the effects of a family member and lineal consanguinity’s physical and mental health. Relatives’ physical and mental health were estimated by three aspects: whether they were suffering from chronic diseases, mental illness, or alcoholism. (3) Results: The study showed that a family member’s physical (OR = 2.303, p < 0.01) and mental health (OR = 5.877, p < 0.05) was related to suicidal ideation, but the association between lineal consanguinities’ physical and mental health and suicidal ideation were not supported. People over 40 years old (OR = 6.528, p < 0.05), from only-child families (OR = 4.335, p < 0.01), with household indebtedness (OR = 2.992, p < 0.001), or difficulty falling asleep (OR = 3.165, p < 0.001) had risk factors of suicidal ideation. (4) Conclusions: The physical and mental health of individuals’ family members are related to their suicidal ideation, and their lineal consanguinities’ physical and mental health are not related to suicidal ideation. These findings imply the different associations between family environment, genetic factors, and suicidal ideation. Family members’ health should be considered as a factor to prevent and control suicidal behaviors, including suicidal ideation.
