Photodynamic therapy vs. imiquimod 5% cream as skin cancer preventive strategies in patients with field changes: a randomized intraindividual comparison study

Abstract: MAL-PDT and IMIQ 5% are safe and well-tolerated treatments that equally prevent development of new AKs in patients suffering from field changes. MAL-PDT treatment appears to be superior in terms of patients' preference.

“…The continuous 75 mW/cm 2 red light protocol was considered due to its standardized use across Europe while the choice of the fractionated 12.3 mW/cm 2 red light protocol was motivated by the FLEXITHERALIGHT Project (http://www.flexitheralight.com/). This French National Research Agency Project focuses on the development of a biophotonic device based on a flexible light emitting textile enabling a fractionated irradiation with a 12.3 mW/cm 2 fluence rate for the PDT treatment of actinic keratosis.…”

“…These issues have a strong impact on the adherence of patients (particularly if they are elderly or frail) to the treatment, and therefore they can influence to a large extent the effectiveness of a treatment in ‘real life' [18]. In addition, the present results may help to identify which therapy may be more suitable for a given subset of patients.…”

“…The 2 treatments equally prevented the development of new lesions on the treated fields during a 12-month follow-up period. The overall patient preference was in favor of MAL-PDT in 59.1% of patients and imiquimod in 40.9% [18]. …”

“…In 2015, imiquimod cream 5 % was compared with methyl aminolevulinate (MAL)-PDT as a field-directed treatment. While they were both found to be safe and equally effective in preventing actinic lesions, patients more often preferred MAL-PDT treatment [64]. That same year, Auriemma et al suggested that oral supplementation with Polypodium leucotomos extract after PDT improved PDT clearance of actinic keratoses, precancerous skin lesions [65].…”
