Photodynamic Therapy Is an Effective Adjuvant Therapy for Image-Guided Surgery in Prostate Cancer

Abstract: Local and metastatic relapses of prostate cancer often occur following attempted curative resection of the primary tumor, and up to 66% of local recurrences are associated with positive margins. Therefore, technologies that can improve the visualization of tumor margins and adjuvant therapies to ablate remaining tumor tissues are needed during surgical resection of prostate adenocarcinoma. Photodynamic agents have the potential to combine both fluorescence for image-guided surgery (IGS) and photodynamic therap… Show more

“…In another study, Doeveren et al used adjuvant meta -tetrahydroxyphenylchlorin-mediated PDT in patients with a malignancy in the head and neck with close or positive resection margins who were not eligible for conventional treatment modalities and they reported that PDT could be applied as adjuvant therapy after surgery ( van Doeveren et al, 2018 ). Wang et al recently proposed that in prostate cancer, PDT might be an effective adjuvant therapy for image-guided surgery ( Wang X. et al, 2020 ). Friedberg et al reported that Foscan mediated PDT may be safely combined with surgery at the established maximally tolerated dose ( Friedberg et al, 2003 ).…”

“…Thus, PDT is a multiple-targeting method. Since the US Food and Drug Administration listed PDT as a new treatment method for the clinical treatment of cancer patients[ 3 ], alone or combined with surgery[ 4 ] and/or chemotherapy[ 5 ], PDT has been applied in large numbers worldwide. For PDT treatment, some Western countries have established relatively systematic treatment plans[ 6 - 8 ].…”

“…Orthotopic PC3pipGFP prostate cancer models were established as previously described using green fluorescence protein (GFP) transfected PC3pip cells 59 . Mice bearing orthotopic PC3pipGFP tumors were injected with 100 nmol/kg of PSMA-1-MMAE-IR700.…”

“…68 Ga-PSMA11 and 18 F-labeled Pylarify have been approved by Food and Drug Administration (FDA) for positron emission tomography (PET) imaging of prostate cancer. We have successfully developed a high affinity PSMA ligand (PSMA-1) 57 , and have utilized it to develop PSMA-targeted PDT agents 58 , 59 . Although our PSMA-targeted PDT conjugates showed effective tumor inhibition, some cancer cells were found to survive and tumor recurrences were eventually seen in immunocompromised mouse models after PDT treatment 58 .…”
