Photodynamic therapy (PDT), a treatment of choice for cancer, induces a photochemical reaction, thereby eradicating tumor cells. This is achieved through the administration of a photosensitizer drug, which is activated with a laser after localization to the tumor mass, and is an approved curative endoscopic ablative treatment for superficial esophageal squamous cell carcinoma (ESCC) in Japan. PDT has been approved for dysplastic Barrett's esophagus and as a palliative treatment for patients with symptomatic obstructive esophageal cancer in US. However, its adverse events and complicated procedure and the development of alternative endoscopic procedures such as endoscopic submucosal dissection, radiofrequency ablation and cryotherapy, have largely limited the practice of PDT in esophageal cancer worldwide. Recently, owing to the invention of second‐generation PDT using talaporfin sodium and diode laser, PDT can be performed with less phototoxicity and therefore has regained popularity in the treatment of ESCC. As a salvage treatment for patients with local failure after chemoradiotherapy (CRT), PDT has shown promising complete response with less phototoxicity and shorter sun shade period. In addition, the efficacy and safety of PDT in patients with local failure of ESCC after CRT were shown in several clinical trials. The direction of the study interest of the next‐generation PDT is the safety and potential expansion of the indications for its application in the future. This review covers the PDT for the treatment of ESCC and dysplastic Barrett's esophagus, with special focus on the role of PDT in practice for esophageal cancer.

Introduction: Photodynamic therapy is a minimally invasive clinical treatment modality for a variety of premalignant and malignant conditions combining a photosensitizing drug, oxygen and light irradiation. Hematoporphyrin is an organic photosensitizer, which mediates inhibition of endothelial cell proliferation and induces apoptosis. Human serum albumin is an endogenous drug carrier for hematoporphyrin. The present study aimed to investigate the hematoporphyrin binding to human serum albumin, which is its transport protein. Material and methods: The chemical reagents were hematoporphyrin (Hp), human serum albumin (HSA) and bovine serum albumin (BSA). In the experiment two techniques were used: spectrofluorimetry and UV-Vis absorption spectrophotometry. Results: The binding sites for Hp were identified in the tertiary structure of HSA by fluorescence quenching technique. The experiment with BSA delivered additional data on Hp-albumin interactions close to Trp135. The participation of tyrosyl residues apart from tryptophanyl ones was discussed. A decrease of the polarity in the binding sites, testifying to possible hydrogen bonding in the binding sites, was also described. The binding and quenching constants Hp-HSA and Hp-BSA were determined as well as the number of binding sites. Conclusions: Hp locates in subdomain IIA in the tertiary structure of HSA. The location in subdomain I close to Trp135 is also possible. Hp is also able to interact within tyrosyl residues.

Objectives Compared to normal cells, malignant cells have a high degree of aerobic glycolysis, also known as the Warburg effect. Therefore, supplementing photodynamic therapy (PDT), an established cancer therapy, with metabolic inhibitors can augment the mitochondrial damage by depleting ATP. To assess the combined impact of the glycolysis inhibitor 2‐deoxy‐D‐glucose (2‐DG) and PDT on apoptosis and autophagy in human breast cancer cells, and examine the molecular basis. Methods Calcium‐AM/PI double staining was used to evaluate cell viability. Reactive oxygen species (ROS), mitochondria membrane potential (MMP), nuclear morphology, and autophagosomes were measured using specific fluorescent markers. In addition, translocation of the apoptosis inducing factor (AIF) from the mitochondria to nucleus was imaged by confocal laser scanning microscopy, and DNA fragmentation was measured using PI staining and comet assay. PGC‐1α expression, oxidative phosphorylation, ATP levels, and autophagy related proteins were detected by qRT‐PCR, seahorse bioscience XFP extracellular flux analyzer, and Western blotting, respectively. Results Compared to with either monotherapy, 2‐DG+PDT resulted in significantly higher cytotoxicity in the three breast cancer cell lines (MDA‐MB‐231, MCF‐7, and 4T1), which was consistent with tumor growth regression trends seen in the 4T1 xenograft model. A synergistic augmentation of mitochondrial dysfunction (in terms of ROS generation, MMP loss, and PGC‐1α down‐regulation) and ATP depletion was seen in cells receiving 2‐DG and PDT. In addition, nuclear translocation of AIF and the subsequent DNA damage indicated that the cytotoxic effects were mediated by a caspase‐independent mechanism, which was relieved by the ROS scavenger N‐acetylcysteine. Autophagy via the AMP‐activated protein kinase (AMPK) was also observed following 2‐DG+PDT, and reversed upon pre‐treatment with the autophagy inhibitor 3‐methyladenine. Conclusions The anti‐cancer effects of 2‐DG+PDT are mediated by both mitochondria triggered apoptosis and AMPK‐mediated autophagy. Lasers Surg. Med. © 2018 Wiley Periodicals, Inc.
