“…All of these centers performed a quandrantectomy to remove the tumor, so the question remains whether lesser surgeries, such as a standard lumpectomy, can lead to similar outcomes when combined with IOERT APBI. IOERT APBI published studies, using lesser surgeries, come from Montpellier [ 99 ], Brussels [ 100 ], Nagoya [ 101 ], and Chapel Hill [ 102 ].…”

“…Sawaki et al [ 101 ] has treated 32 patients, 88 % with IDC, with IOERT APBI at Nagoya. Though the numbers are small, and the study was primarily designed to assess the toxicity of the procedure, it is the fi rst report of single-fraction IOERT in the Asian population, which has smaller, denser breasts than those of other ethnic groups.…”

“…In Asian breast cancer patients 21 Gy was recommended as a result of phase I study, which had been used a scheme of dose-escalation from 19, 20, and 21 Gy. (Sawaki et al, 2009), and subsequent phase II study indicated feasibility at 21 Gy (Sawaki et al, 2011). In this technique a disk should be needed to reduce the spread of irradiation to lung and heart.…”

“…Data from observational series, small trials, and a systematic review have been reported and formed the basis for the two pivotal, international, randomized trials [722]. In fact, prior to the development of one of the randomized trials, the IORT technique was assessed in a phase II study in which IORT was delivered as a tumor bed boost and was followed by conventional, whole breast EBRT.…”
