Pharmacotherapy for the Treatment of Overweight and Obesity in Children, Adolescents, and Young Adults in a Large Health System in the US

Abstract: Lifestyle modifications focused on diet, physical activity, and behavior have a modest impact on weight reduction in children, adolescents, and young adults (YA) with overweight and obesity. Several anti-obesity medications (AOMs) have been approved by the Food and Drug Administration (FDA) for use among adult patients with a body mass index (BMI) ≥27 kg/m 2 and at least one obesity-related illness. “…But the effect of prescribing metformin with exercise in children and adolescents have on the rate of gain in these fitness outcomes is largely unknown in boys and girls. With emerging literature suggesting that off label or prophylactic use of metformin may be effective for weight management and obesity prevention in adolescents (54,71,127) more children may be provided metformin and recommended to exercise. This raises potential concern toward altered maturation growth rates and cardiometabolic risk during youth as well as then for later in life health risk compared with youth advised to exercise only with proper nutrition (54,71).…”

“…Studies have found significant improvement when topiramate is used as adjunctive therapy for refractory bipolar disorder (Mula et al 2006). A small study found that adjunctive topiramate therapy for bipolar disorder type I/II in the pediatric population led to a 62% improvement (DelBello et al 2002).…”

“…This combination medication has been shown to be safe and effective in adolescents (Hsia et al 2020). Appetite suppression caused by topiramate may be due to activation of the GABA-A receptor as well as antagonism of the kainite/AMPA glutamate receptors that stimulate appetite in the lateral hypothalamus (Arnone 2005;Czepiel et al 2020). Topiramate has also been shown to inhibit fat deposition by modulation of lipoprotein lipase (Richard et al 2000).…”
