Pharmacological interventions for people with depression and chronic physical health problems: systematic review and meta-analyses of safety and efficacy

Abstract: Antidepressants are efficacious and safe in the treatment of depression occurring in the context of chronic physical health problems. The SSRIs are probably the antidepressants of first choice given their demonstrable effect on quality of life and their apparent safety in cardiovascular disease.

“…Drugs that inhibit FAAH activity prolong and enhance the effects of these endogenous CB 1 and PPAR-α ligands when and where they are produced, and might therefore be better tolerated than exogenous ligands such as THC (Piomelli et al, 2006). This general strategy has been applied successfully, for example, with medications that increase endogenous levels of other neurotransmitters such as serotonin (Taylor et al, 2011) and dopamine (Godfrey, 2009).…”

“…Depression, in view of its clinical implications in the prognosis of medical disorders regardless of its actual severity, has been equated to ‘bad cholesterol' and the use of antidepressant drugs to statins, which should not be refused to anyone as a preventive or treatment measure - well beyond their original indications [197]. While antidepressant drugs were found to be effective for treating major depression in the setting of medical disease [198,199], their actions on improving medical outcomes have not been demonstrated [200,201] and may rather involve side effects, interactions, and the likelihood of developing iatrogenic comorbidity [118,190,197,202]. …”

“…Whether an attempt was made to pool results across indications and whether potential variation in treatment effects between different indications was assessed. Pooled across different indications without considering between-indication variation 21 (40) Pooled across indications for some outcomes (eg, adverse events) but not for the others (eg, effectiveness outcomes) 4 (8) Quantitatively examined variation/heterogeneity between indications with or without pooling across indications 11 (21) No pooling across different indications and no assessment of variation/heterogeneity between indications (quantitative results reported/displayed separately for each study/indication)…”

“…Conditions can also be included to test the hypothesis that effects differ according to theoretical construct, for example, clean vs. contaminated surgery in the aforementioned example. Likewise, the effects of antidepressants and cognitive behavior therapy have been compared across a range of conditions expressly to explore the theory that their effects are linked through a common pathway [21,22].…”
