Pharmacological enhancement of disc diffusion and differentiation of healthy, ageing and degenerated discs

Abstract: Degenerative disc disease (DDD) is still a poorly understood phenomenon because of the lack of availability of precise definition of healthy, ageing and degenerated discs. Decreased nutrition is the final common pathway for DDD and the status of the endplate (EP) plays a crucial role in controlling the extent of diffusion, which is the only source of nutrition. The vascular channels in the subchondral plate have muscarinic receptors but the possibility of enhancing diffusion pharmacologically by dilation of th… Show more

“…Diffusion study offers a powerful means of understanding the circumstances of disc nutrition. As Rajasekaran et al have previously described, the pattern with decreased diffusion indicates decreased nutrition supply to the disc, while the pattern with vascularization phenomenon indicates very severe IDD with totally abnormal diffusion [4,21,25]; both of these indicated that the endplate nutritional pathway was interfered with and diseased as a result of bone cement intervention.…”

“…Such a process would affect the loading capabilities and segmental motion of the involved segment, thereby contributing not only to disc degeneration of the involved segment, but, because the distribution of stress forces will be altered, the adjacent level may be affected to some degree as well (49)(50)(51)(52)(53)(54).…”

“…Decreased nutrition is the final common pathway for degenerative disc disease and the status of the endplate plays a crucial role in controlling the extent of diffusion and is the only source of nutrition. 15 A recurrence rate of 5% to 15% for LDH has been reported. [1][2][3][4][5] Differentiation of recurrent disc herniation from scar formation will allow for improved treatment choices and selection of patients who may benefit from a second surgery.…”
