Pharmacological and non-pharmacological treatments and outcomes for new-onset atrial fibrillation in ICU patients: the CAFE scoping review and database analyses

Abstract: Background New-onset atrial fibrillation occurs in around 10% of adults treated in an intensive care unit. New-onset atrial fibrillation may lead to cardiovascular instability and thromboembolism, and has been independently associated with increased length of hospital stay and mortality. The long-term consequences are unclear. Current practice guidance is based on patients outside the intensive care unit; however, new-onset atrial fibrillation that develops while in an intensive care unit dif… Show more

“…However, optimal amiodarone dosing regimens are still unknown (46), and clinicians are often under the false impression of neglectable side-effects. (47) Interestingly, we found an increasing number of mCV attempts with rising lactate levels: 16% in lac < 1.1mmol/L vs. 26% in lac > 2.3mmol/L. Amiodaron was less effective with rising lactate levels: 63% SR in lac < 1,1mmol/L vs. 49% SR in lac > 2.3mmol/L.…”

“…1,21–24 In patients with sepsis, the incidence of acute AF varies with the severity of sepsis, with an incidence of 8% to 10% in sepsis, 6% to 22% in severe sepsis, and 23% to 44% in septic shock. 1,21,23,24 Acute AF is associated with longer length of hospitalization, 23–25 greater morbidity 15,21,22,26 and mortality, 21,23,26–28 and high rates of recurrent AF. 1,15…”

“…Among class III agents, amiodarone, a multichannel blocker, is highly effective and widely used in ICU patients despite its known toxicity [138][139][140][141]. Although it prolongs the QT interval, it seldom causes polymorphic ventricular tachycardia (Torsade des pointes) [133,142].…”
