Pharmacokinetics of a single, large dose of cholecalciferol

Abstract: Cholecalciferol (100,000 IU) is a safe, effective, and simple way to increase calcidiol concentrations. The dosing interval should be < or =2 mo to ensure continuous serum calcidiol concentrations above baseline. This trial was registered at www.clinicaltrials.gov as #NCT00473239.

“…On the other hand, with HyD at a dose of 20 mg, the 25(OH)D threshold of 30 ng/mL was achieved in all women already at 35 days of treatment, and after 3 to 4 months, the 25(OH)D level rose to a mean concentration maximum of 69.5 ng/mL. As demonstrated by Ilahi and colleagues, (27) 3 ; however, only with a much higher oral bolus of 2500 mg (100,000 IU).…”

“…The authors did not provide information as to how many participants achieved a value of 480 nmol/l. Vitamin D3 in a single oral dose of 100 000 U given to adult men and women was safe and effective in raising 25OHD to 480 nmol/l in 93% of the participants (Ilahi et al, 2008). Mean 25OHD stayed above 80 nmol/l for up to 70 days and approximately above 70 nmol/l for 90 days.…”

“…It should first be noted that vitamin D supplementation was delivered once a month in our study and that DSLs once vitamin D supplementation had been started were measured a few days before a monthly intake. However, we know from pharmacokinetic studies that after a single dose of 100,000 IU of cholecalciferol [Ilahi et al 2008] (i.e. the same dose as in our study), the DSL decreases only slightly for the second half of the month following the intake, and that, with regular monthly intakes of vitamin D when compared with analogous weekly or daily intakes [Ish-Shalom et al 2009 , (2) in compliance with vitamin D intake or even (3) in changes in the lifestyle habits of some patients, once they were aware of a potential role of the environment in the course of their disease.…”
