SummaryCurrent treatment regimens for haematological autoimmune diseases are relatively non-selective and are often associated with considerable toxicity. Recently, it has become clear that B cells play a key role in both the development and perpetuation of autoimmunity, suggesting that B-cell depletion could be a valuable treatment approach for patients with autoimmune diseases. This article reviews data supporting the use of rituximab -an anti-CD20 monoclonal antibody that specifically depletes B cells -in four key autoimmune haematological disorders: idiopathic thrombocytopenic purpura (ITP); autoimmune haemolytic anaemia (AIHA); acquired haemophilia; and thrombotic thrombocytopenic purpura (TTP). Although treatment of ITP, AIHA, acquired haemophilia and TTP with rituximab is still relatively uncommon, results from case series and small phase II trials indicate that patients of all ages can respond to rituximab, irrespective of the number or type of prior treatments that they have received. Moreover, patients with these diseases receiving rituximab experienced predominantly mild adverse events, with only a few serious adverse events reported. These data suggest that rituximab provides an effective and well-tolerated alternative treatment option for patients with ITP, AIHA, acquired haemophilia and TTP, many of whom have limited treatment choices.

Національний фармацевтичний університет ДУ «Інститут патології крові та трансфузійної медицини НАМН України»* Клініко-економічний аналіз різних стратегій лікування гемофілії А Хоча гемофілія є рідкісним захворюванням, його лікування є високовартісним, а витрати в основному пов'язані із вартістю препаратів концентратів факторів згортання крові. Мета роботи -оцінка клініко-економічної доцільності застосування rFVIIa (препарат НовоСевен ® ) для ліку-вання пацієнтів з гемофілією у порівнянні зі схемами, що ґрунтуються на застосуванні АРСС (препарат Фейба). Матеріали та методи. Клініко-економічний аналіз проведено методом «загальна вартість захворювання» з використанням моделювання шляхом побудови дерева рішень. У даному дослідженні визначалися лише прямі витрати на фармакотерапію. Вартість упаковки кожного з аналізованих препаратів відповідала задекларова-ній ціні станом на грудень 2016 року. Результати. Аналітична модель «дерева рішень» розроблена для розрахунку витрат на амбулаторне лікуван-ня легких та помірних кровотеч. За рахунок більш високої клінічної ефективності використання rFVIIa в якості першої лінії терапії дозволяє зекономити від 32 % до 42 % при лікуванні одного пацієнта, що в умовах сучас-ного українського фармацевтичного ринку складає від 170326,4 грн до 252990,4 грн (при адаптованому варі-анті терапії), та до 291743,1 грн при базовому варіанті. При використанні rFVIIa в якості першої лінії терапії нижчими виявилися витрати протягом усього життя -приблизно на 10 % нижче, ніж при використанні АРСС. Висновки. Застосування препарату НовоСевен ® є економічно більш доцільним і дозволяє як покращити ре-зультати лікування, так і більш раціонально витрачати грошові ресурси на лікування цих хворих. Although hemophilia A is a rare disease, its treatment is costly, and expenses are mainly related to the cost of factor VIII concentrates. Aim. To evaluate the clinical and economic feasibility of rFVIIa (NovoSeven ® ) for the treatment of patients with hemophilia A compared to the schemes based on the use of APCC (Feiba). Matherials and methods. Clinical and economic analysis was conducted by the "total cost of illness" method using modeling by building a "decision tree". This study identifies only the direct costs of pharmacotherapy. The cost of a vial of each of the concentrates analyzed meets the declared value as of December 2016.Results. The analytical model of the "decision tree" was designed to calculate the costs for the outpatient treatment of mild and moderate bleedings. Due to higher clinical efficacy the use of rFVIIa as the first-line therapy saves 32 % to 42 % in the treatment of a patient; in the conditions of the modern Ukrainian pharmaceutical market it is from UAH 170,326.4 to UAH 252,990.4 (with an adapted variant of therapy) and UAH 291,743.1 in the basic variant. When using rFVIIa as the first-line therapy the costs for life appeared to be lower by approximately 10 % than when taking APCC. Conclusions.
