Phacoemulsification with intraocular lens implantation in patients with uveitis

Abstract: With careful patient selection, appropriate pre-operative preparation, diligent surgery and close post-operative supervision, phacoemulsification with intraocular lens implantation is safe and effective in the great majority of eyes with uveitis.

“…Notwithstanding the accuracy imparted by angiography, this difference is large and may in part be explained by casemix differences, especially by our inclusion of patients with FHU. A lower incidence (2.3%) has also been reported 25 in a cohort of 86 eyes with uveitis undergoing phacoemulsification by one of us (NPJ), albeit with shorter follow-up. The progression of macular disease is difficult to quantify in this study.…”

“…The early postoperative results of 39 of these patients have been previously reported. 18,25 This study includes eyes with Fuchs' heterochromic uveitis. In general, reports on cataract surgery in this disease have assumed a fundamental difference between FHU and other forms of uveitis, and a better outcome is often claimed.…”

“…3,5,17 Previous studies have re- ported short and long-term visual improvement of ≥ 2 lines in up to 93% of patients. [18][19][20][21] In our study, 72.4% of the eyes showed ≥ 2 Snellen lines of visual improvement at the final visit. Fuchs uveitis syndrome was associated with the best visual outcomes in our series.…”

“…Many published series have reported excellent visual results after extracapsular cataract extraction with a rigid one-piece IOL, [1][2][3] as well as phakoemulsification using a foldable IOL. 4,5 This has been because of a combination of an improvement in microsurgical techniques including small incision surgery, the appropriate timing of the surgery, and adequate pre-, periand postoperative control of the intraocular inflammation. The biocompatibility of IOLs, or their ability to remain inert and minimise host reactions, is also of paramount importance.…”
