Phacoemulsification tip vacuum pressure: Comparison of 4 devices

Abstract: Although there were some variations in vacuum pressures among phacoemulsification machines, particularly when an aspiration bypass tip was used, these discrepancies are probably not clinically significant.

“…[17][18][19][20] Contrary to expectations and paradoxically, our results show that the Venturi pump may have a marginally lower risk of capsular breakage than the peristaltic pump, although this difference did not quite reach statistical significance (P ¼ .0128). Once we included tip occlusion data in the comparison, the difference between breakage rates for peristaltic and Venturi systems clearly was not significant.…”

“…To reduce the amount of total US energy dissipated in the eye, some surgeons use high hydrodynamic parameters (vacuum and flow rate) to accelerate surgery (7) . Several new phacoemulsification strategies and new devices were developed to preserve the corneal endothelium by attenuating the deleterious effects of US (8) . Some surgeons have attempted to alleviate the corneal load by varying the position of the phacoemulsification tip (4,9) .…”

“…The human eye-bank model is the closest to the clinical situation we can reasonably expect to measure, and by knowing that the parameters we used are comparable between machines from our previous work, [5][6][7][8] we could see the true impact of machine technology and Cruise Control on surge. The major limitation of this experiment is that it was not a clinical study; however, repeating this study during surgery (all in the same eye) is an impossible clinical scenario.…”

“…In previous studies, [5][6][7][8] we showed how certain adjustable variables, such as the aspiration vacuum, aspiration flow rate, and phaco power, are not comparable among machines. We also found that the machines show considerable variance to surge with Cruise Control, with a small but statistically significant impact on the Sovereign unit (AMO) but not the Millennium peristaltic unit (Bausch & Lomb, Beverly Hills, CA).…”
