PET imaging of focal demyelination and remyelination in a rat model of multiple sclerosis: comparison of [11C]MeDAS, [11C]CIC and [11C]PIB

Abstract: This study showed that PET imaging of demyelination and remyelination processes in focal lesions is feasible. Our comparison of three myelin tracers showed that [(11)C]MeDAS has more favourable properties for quantitative PET imaging of demyelinated and remyelinated lesions throughout the CNS than [(11)C]CIC and [(11)C]PIB.

“…27 To overcome the problem, SUV normalized by reference region has been proposed as a more accurate quantification method for [ 11 C]PIB static imaging. 28 This was not applicable in our case since the reference region is extracted through tissue kinetic clustering thus requiring dynamic imaging.…”

“…Sites of inflammation in MS are often hypoxic and acidic (33, 52, 53) and, accordingly, tissue from MS lesions exhibits hypoxic markers, including increased expression of glucose transporters (GLUTs) and monocarboxylate transporters (MCT), compared with healthy patients (54), most likely due to increased glycolysis of activated MPs. The increased avidity for glucose by MS lesions can be exploited for diagnostic purposes, since these lesions uptake substantial amounts of the glucose analog [ 18 F]-fluorodeoxyglucose, which can be visualized by positron emission tomography during neuroimaging exams (55). In line with these findings, tissue hypoxia and the corresponding increased expression of HIF-1α develops rapidly in response to inflammation in white and gray matter of animals subject to EAE.…”

“…In MS, this low uptake of PiB in the cerebellar region would complicate the detection of lesions since demyelinated lesions show decreased signal compared to intact myelin. A study comparing different radio tracers for PET by de Paula Faria et al [33] found that the radio tracer MeDAS has more favorable properties than PiB for quantitative PET imaging of demyelinated and remyelinated lesions throughout the CNS. MeDAS showed a higher uptake than PiB in white matter regions, and in the healthy cerebellum and brainstem it was significantly higher than PiB.…”
