PET/CT-Based Characterization of 18F-FDG Uptake in Various Tissues Reveals Novel Potential Contributions to Coronary Artery Disease in Psoriatic Arthritis

Abstract: Background and ObjectivesPsoriasis is a heterogeneous inflammatory disease that involves the skin, joints, liver, heart, and other organs. Psoriatic arthritis (PsA) is associated with cardiovascular disease (CVD), but the relative contributions of inflammatory and metabolic dysregulation to CVD are incompletely understood. We set out to discover novel potential contributors to CVD in PsA patients by comprehensively phenotyping a cohort of PsA patients using these advanced technologies.MethodsIn this cross-sect… Show more

“…The medical records of the patients were explicitly searched to ascertain the presence of CV risk factors: (1) dyslipidemia (physician diagnosis, or the use of lipid-lowering medication, or at least one factor: total cholesterol > 200 mg/dL, triglycerides > 150 mg/dL, HDL-cholesterol < 40 mg/dL in men or < 50 mg/dL in women, or LDL-cholesterol > 130 mg/dL), (2) hypertension (physician diagnosis and/or use of anti-hypertensive medications), (3) obesity (BMI ≥ 30 kg/m 2 ), (4) currently active smoking, and (5) diabetes mellitus (physician diagnosis, or glycemia > 126 mg/dL, HbA1c > 6.5%, or glucose-lowering drugs or insulin therapy) [ 2 , 30 32 ].…”

“…In addition, imaging techniques showed sites of inflammation in extracutaneous tissues [ 28 30 ]. In particular, Mehta and coworkers evidenced vascular inflammation in patients with psoriasis, through F-fluorodeoxyglucose positron emission tomography computed tomography (PET-CT) [ 31 , 32 ].…”

“…Computed tomographic angiography (CTA) is currently routinely used for the noninvasive evaluation of epicardial coronary arteries and large vessels. Compared with age-matched controls, patients with RA had a greater prevalence of asymptomatic CAD, with higher coronary calcium score (CCS), more frequent multivessel disease, as well as more high-risk plaques [164]. Furthermore, the use of biologic disease-modifying anti-rheumatic drugs (bDMARDs) has been associated with reduced CVD risk based on CTA findings [165].…”
