BackgroundIndividuals who are sick and unable to work may receive wage replacement benefits from an insurer. For these provisions, a disability evaluation is required. This disability evaluation is criticised for lack of standardisation and transparency. The International Classification of Functioning, Disability and Health (ICF) was developed to express the situation of people with disability. We discuss potential benefits of the ICF to structure and phrase disability evaluation in the field of social insurance. We describe core features of disability evaluation of the ICF across countries. We address how and to what extent the ICF may be applied in disability evaluation.DiscussionThe medical reports in disability evaluation contain the following core features: health condition, functional capacity, socio-medical history, feasibility of interventions and prognosis of work disability. Reports also address consistency, causal relations according to legal requirements, and ability to work. The ICF consists of a conceptual framework of functioning, disability and health, definitions referring to functioning, disability and health, and a hierarchical classification of these definitions. The ICF component ’activities and participation’ is suited to capture functional capacity. Interventions can be described as environmental factors but these would need an additional qualifier to indicate feasibility. The components ‘participation’ and ‘environmental factors’ are suited to capture work requirements. The socio-medical history, the prognosis, and legal requirements are problematic to capture with both the ICF framework and classification.SummaryThe ICF framework reflects modern thinking in disability evaluation. It allows for the medical expert to describe work disability as a bio-psycho-social concept, and what components are of importance in disability evaluation for the medical expert. The ICF definitions for body functions, structures, activity and participation, and environmental factors cover essential parts of disability evaluation. The ICF framework and definitions are however limited with respect to comprehensive descriptions of work disability.

In the International Classification of Functioning, Disability and Health (ICF), data on environmental and personal factors, the so-called facilitators or barriers, can be gathered. The World Health Organization (WHO) has not classified personal factors. This is justified with the cultural differences among countries. The German society for social medicine and prevention (DGSMP) has performed pioneering work and has provided a classification draft. The aim of this study is to validate this draft in the context of oncological rehabilitation of breast cancer patients in Germany. 21 experts from 8 rehabilitation centers and 2 breast cancer support groups in Germany were interviewed about personal factors in a 2-round survey using the Delphi technique. The expert group included doctors, nurses, therapists, psychologists with experience in rehabilitation of breast cancer patients and breast cancer patients. The responses of the first round were linked to the DGSMP draft. The degree of fit was calculated by means of the kappa statistic. In the second round, the focus was on the agreement of the personal factors determined in the first round. The experts identified 244 factors out of which 203 (83.2%) were linked to the DGSMP draft. There are 77 categories in the DGSMP draft; 43 of them were included by the experts in the first round and 34 were not mentioned (kappa coefficient 0.776; 95% bootstrapped confidence interval 0.712-0.817). In the second round, 34 of the 43 named DGSMP categories achieved an agreement of at least 75% among the experts. The draft of the DGSMP working group is a comprehensive collection of facilitators and barriers in breast cancer rehabilitation and should be integrated reasonably in rehabilitation. More research on personal facilitators and barriers has to be carried out to show their concrete influence on functioning ability of breast cancer patients.

By the "Second Bill to Strengthen Long-Term Care", a new concept of long-term care dependency will be introduced, valid from 2017. Long-term care dependency according to Social Code XI will be defined covering more aspects than today. Therefore, the working group "Nursing Care" of the division "Social Medicine in Practice and Rehabilitation" in the German Society for Social Medicine and Prevention presents their results after working on the social medicine perspective of the definition and prevention of long-term care dependency. Both the definition and strategies to prevent long-term care dependency are systematically taken into consideration from the point of view of social medicine on the basis of the International Classification of Functioning, Disability and Health (ICF), as long-term care dependency means a defined condition of disability. Both the current and the new concept of long-term care dependency focus activity limitations. The perspective of social medicine considers the interactions of health condition, its effects on daily activities and personal as well as environmental factors. From this point of view approaches for social benefits concerning prevention and rehabilitation can be identified systematically so as to work against the development and progression of long-term care dependency. The reference to the ICF can facilitate the communication between different professions. The new "graduation" of long-term care dependency would allow an international "translation" referring to the ICF. Experts from the field of social medicine as well as those of nursing care, care-givers and nursing researchers have in common the objective that persons in need of nursing care can participate in as many aspects of life of importance to them in an autonomous and self-determined way. The point of view of social medicine on long-term care dependency is fundamental for all occupational groups that are involved and for their successful cooperation.
