Personal cell therapy for interstitial cystitis with autologous stromal vascular fraction stem cells

Abstract: Background: The objective of this study was to evaluate whether autologous stem-cell-based therapy may mitigate the symptoms of interstitial cystitis. Methods: Stromal vascular fraction (SVF) rich in stem cells and derived from autologous adipose tissue was deployed into 109 men and women with interstitial cystitis/painful bladder syndrome as a surgical procedure. This stem-cell-rich biologic product was injected both systemically and regionally into pelvic floor targets. Patients were queried about quality of… Show more

“…The Ustem kit is a commercial product designed to simplify the separation process. The SVF contain stem cells and cytokines, which are associated with regenerative, antinociceptive, immunomodulatory, and anti-inflammatory effects [10]. Thus, the SVF has several advantages over pure ADSCs, as the SVF has a distinct and heterogeneous cellular composition compared to that of MSCs, The SVF has demonstrated superior therapeutic results in several preclinical studies [29][30][31][32].…”

“…Currently, an SVF is usually procured and isolated from subcutaneous adipose tissue via liposuction [ 10 ]. More recently, perirenal adipose tissue has been proposed as a potential cell source for regenerative medicine [ 11 ].…”

“…To date, only 1 clinical trial has assessed the efficacy of stem cells for IC/BPS. In 2019, Lander et al [47] investigated the effect of combined intravenous and local injections of autologous stromal vascular fraction (SVF) into the pelvic floor in 91 women and 18 men with IC/BPS. Although that study demonstrated that SVF, as an autologous personalized regenerative strategy, showed good safety and efficacy and may have the potential to alleviate IC/BPS, the major limitations were the lack of a control group, inconsistent transplantation cell number, inconsistent cell injection routes in male patients, lack of information on the molecular mechanisms of SVF therapy, and the absence of long-term data.…”

“…Recently, the potential applications of regenerative medicine, such as platelet-rich plasma (PRP) and stem cells, have been shown to be beneficial to treatment of IC/BPS in some preclinical and few clinical studies. 9–12 PRP, which contains several growth factors and signal proteins released by actively degranulated platelets, acts on available cells to begin the tissue repair and regeneration processes. 9,10 Nanofat, which is a stromal vascular fraction (SVF) obtained from mechanical emulsification and filtration of adipose tissue and contains a heterogeneous population of stem/progenitor cells and extracellular matrix, is a therapeutic paradigm in regenerative medicine.…”
