Persistent Lower Limb Deformities Despite Amelioration of Rickets in X-Linked Hypophosphatemia (XLH) - A Prospective Observational Study

Abstract: BackgroundGait deviations, lower limb pain and joint stiffness represent key symptoms in patients with X-linked hypophosphatemia (XLH, OMIM 307800), a rare disorder of mineral homeostasis. While the pathomechanism for rickets is well understood, the direct role of PHEX (Phosphate-regulating neutral endopeptidase) deficiency in non-rachitic features including complex deformities, skull and dental affections remains unclear. FGF23-inhibiting antibody treatment can normalize serum phosphate levels and to improve … Show more

“…Our study cohort showed increased internal tibial malrotation similar to recent reports on children and adults with XLH ( 4 , 5 , 23 ) associated with gait disturbances. Moreover, maltorsion of the tibia also leads to increased and abnormal intra-articular contact forces leading to accelerated arthritis development of the ankle due to the change of joint biomechanics ( 13 ).…”

“…Diese sagittalen Fehlstellungen treten jedoch nicht bei allen PatientInnen und in sehr unterschiedlichem Ausmaß auf. In transversaler Ebene ist nach rezenter Forschung einer verstärkten Außendrehung (Maltorsion) des Femurs [8,9] und verstärkte Innendrehung der Tibia [4,8,10] vorliegend (▶Abb. 1).…”
