Persistent hemolytic disease of the fetus and newborn (HDFN) associated with passive acquisition of anti‐D in maternal breast milk

Abstract: Although anti-D is a significant cause of HDFN through placental transfer of antibody, exacerbation of the condition through breast milk antibodies is rarely described. The current case highlights the possibility of this occurring. Discontinuation of maternal breast milk feedings should be considered in infants with HDFN who do not respond to standard treatment.

“…Here, we report a replicable methodology for testing for the presence of RBC antibodies in fresh maternal breast milk in infants with clinically significant, prolonged HDFN. Transmission of clinically significant amounts of maternal antiplatelet IgA antibodies via breast milk has previously been reported, with maternal RBC antibody transmission in humans only remaining a theoretical concept until two recent cases reports and now our case series.…”

“…This infant had prolonged, persistent anemia and hyperbilirubinemia that recovered after cessation of breastfeeding. 13 These cases highlight an association between maternal breastfeeding, presence of RBC antibodies against inherited paternal antigens, and prolonged HDFN.…”

“…In a recent editorial, the case is made that transfusion of Group A plasma in recipients of unknown ABO blood group violates the precautionary principle. 1 We disagree, but not necessarily because Group A plasma is the ideal resuscitation choice. The comparison being made is with Group AB plasma as the time-honored "universal donor" plasma, because clinically evident hemolytic reactions are not seen with AB plasma, regardless of recipient ABO blood group.…”

“…Es finden sich zu der Fragestellung der Übertragung erythrozytärer Allo-Antikörper durch Stillen nur einzelne Fallberichte. In diesen werden erythrozytäre Antikörper (Anti-D, Anti-K1 und Anti-Jk(a)) über Gelkarten- oder Röhrchentechnik in der Muttermilch nachgewiesen 8 , 9 , 10 , 11 . Eine enterale Übertragung auf das Neugeborene wird allerdings nicht bewiesen.…”
