Persistent exudative retinal detachment after photodynamic therapy and intravitreal bevacizumab injection for multiple retinal capillary hemangiomas in a patient with von Hippel–Lindau disease

Abstract: Photodynamic therapy (PDT) has been used in treating peripheral retinal capillary hemangioma (RCH) with satisfactory results. We report a rare case of von Hippel-Lindau (VHL) disease with three large peripheral RCHs, treated with PDT and intravitreal bevacizumab injection (IVB), who developed persistent bullous exudative retinal detachment (RD) despite significant tumor regression. The patient is a sporadic case of VHL disease, with a de novo nonsense mutation in codon 161 with C → T transition at nucleotide p… Show more

“…According to Tsai et al, corticosteroids seem to be more effective in reducing exudation than bevacizumab. 155 Walker et al described a VHL patient with rebound neovascularization after intravitreal bevacizumab injection, again without effect on tumor volume. 156 Also, the systemic use of bevacizumab has been tested.…”

“…Combination therapy, such as that for the small retinal hemangioblastoma in von Hippel-Lindau disease, is probably one of the best treatment choices for VPT. 19 Two of our cases had VT and tumor resection to remove the tumor and ERM at the same time. One had PDT plus IVI of ranibizumab before the surgery in attempt to decrease the tumor size.…”
