Persistent delta antigenaemia in chronic delta hepatitis and its relation with human immunodeficiency virus infection

Abstract: The prevalence of persistent hepatitis delta (HD) antigenaemia and associated factors in patients with chronic infection with the hepatitis delta virus (HDV) were investigated. Among 157 consecutive patients known to be carriers of hepatitis B surface antigen (HBsAg), 36 (23%) had one serum marker of HDV infection (anti-HD and/or HDAg). Nine of the patients with an HDV marker were HDAg positive, including three who were anti-HD negative. A follow-up over a mean period of 13 months showed that five of five pati… Show more

“…Although the definite evidence of T cells controlling HDV infection in humans is inadequate, an observation showed that HDV viremia was highly enhanced in human immunodeficiency virus-infected patients whose circulating CD4 ϩ T cells numbers were greatly reduced. 50 Another recent study revealed that the T-cell response to HDAg is related to the activity of HDV-induced liver disease. 13 These findings support the necessity of T-cell immunity to modulate the disease course of HDV infection.…”

“…2,12 The IgG antibody is not protective; it is present in all immunocompetent patients with HDV infection, coexisting with active HDV infection and hepatitis D, and may not be raised in immunocompromised patients who acquire HDV such as patients with AIDS. 15 The IgG antibody is usually short lived in coinfected patients who clear HBV HDV. The exception is drug addicts; in a retrospective analysis of blood from 27 Swedish drug addicts drawn 5 to 13 years after the acute episode of HDV coinfection, 26 were clear-cut positive and one still gave a borderline positive result for anti-HD.…”

“…Occasionally, the HDAg can be detected in serum of patients with an impaired immune system who do not raise antibodies, such as patients with acquired immune deficiency syndrome (AIDS) or transplanted patients under pharmacologic immunosuppression. 15,24 HDV RNA…”

“…Other methodologies include detection of HDV RNA in serum by direct nucleic acid hybridization or with reverse transcriptase-polymerase chain reaction (RT-PCR) (40). PCR detection of HDV has shown it to be a potential problem in patients with the human immunodeficiency virus (HIV) (41). Liver biopsies may also be examined by immunofluorescence, immunoperoxidase, or in situ hybridization for the presence of HDV.…”
