A Comparison of Self-Reported Emotional and Trauma-Related Concerns Among Sexually Abused Children With and Without Sexual Behavior Problems

Numerous studies document concomitant features of sexual behavior problems (SBPs) among children 12 years of age or younger, but rarely does research involve child self-report assessments. This study provides the most comprehensive examination to date of self-reported concerns among children with SBP, using a large sample (N = 392) of clinically referred participants who reported sexual abuse histories. Children between the ages of 8 and 12 were categorized as demonstrating SBP (n = 203) or not demonstrating SBP (n = 189) as determined by scores on the Child Sexual Behavior Inventory. Children completed the Trauma Symptom Checklist for Children, and caregivers completed the Child Behavior Checklist. Self-reports of children showed that those with SBP reported significantly greater concerns in all areas, including sexual preoccupation and sexual distress, than their peers not demonstrating SBP. Caregivers of children in the SBP group reported greater concerns of internalizing and externalizing problems than the caregivers of children who did not have SBP. Implications for clinical practice and future research are discussed. Specifically, it is recommended that future research improve on the manner in which sexual abuse and SBPs were defined and assessed.

En protection de l’enfance, les enfants ayant des comportements sexuels problématiques (CSP) suscitent parfois des malaises chez les intervenants et provoquent de l’inquiétude en raison des torts qu’ils peuvent causer. Prenant appui sur un modèle explicatif adapté par Boisvert et ses collaborateurs, la présente étude visait à cerner les facteurs qui distinguent les enfants présentant des CSP des autres enfants pris en charge en protection de l’enfance. Des analyses secondaires ont été réalisées à partir des données de l’Étude d’incidence québécoise de 2008 sur les situations évaluées en protection de la jeunesse. L’échantillon comprenait 1020 enfants de 2 à 12 ans ayant fait l’objet d’un signalement retenu et fondé. Les enfants avec CSP (n = 72) ont été comparés aux enfants sans CSP (n = 948) sur un ensemble de variables personnelles et familiales au moyen d’analyses univariées, puis multivariées. Le modèle final de régression logistique révèle que les enfants avec CSP sont plus susceptibles de manifester une variété de problèmes de fonctionnement, d’avoir fait l’objet d’un signalement fondé pour agression sexuelle et d’avoir un père ou une mère qui a vécu un placement dans l’enfance. À l’inverse, ils sont moins susceptibles d’avoir fait l’objet d’un signalement fondé pour mauvais traitements psychologiques. La discussion traite de l’incidence des résultats pour l’intervention en contexte de protection.In addition to sometimes making child protective service workers ill at ease, sexual behaviour problems (SBPs) in children are a source of concern due to the harm these behaviours can potentially cause. Based on an explanatory model adapted from Boisvert et al., this study set out to identify the factors differentiating children with SBPs from other children cared for by child protective services. Secondary analyses were conducted with data obtained from the Quebec Incidence Study on situations investigated by child protective services in 2008 (Étude d’incidence québécoise sur les situations évaluées en protection de la jeunesse en 2008). The sample consisted of 1,020 children aged 2 to 12 with a substantiated report to child protective services. Children with SBPs (n = 72) were compared to children without SBPs (n = 948) based on several personal and familial variables using univariate, then multivariate analyses. The final logistic regression model indicates that children with SBPs are more likely to show a variety of adjustment problems, be reported for substantiated sexual abuse, and have a parent who underwent a placement episode in childhood. Conversely, they are less likely to be reported for substantiated psychological maltreatment. The discussion covers the implications of the findings for intervention in a protective services context
