Peroral Dual Scope for Natural Orifice Transluminal Endoscopic Surgery (NOTES) Gastrotomy Closure

Abstract: This transoral dual-scope clipping method of gastrotomy closure after NOTES, as well as the general concept of employing 2 separate instruments at the same time perorally, is proven technically feasible, safe, and effective in this model.

“…Consequently, in recent years, numerous devices have been developed to stitch, staple, or clip GI wall defects from an endoluminal approach. 1,4,6,8,[12][13][14][15][16][17][18][19][20] One of these novel devices is the OTSC system, which has recently become commercially available in Europe and is in the regulatory process in the United States. The system consists of a nitinol alloy clip that is preloaded on a transparent 10-, 12-, or 14-mm applicator cap mounted on the endoscope tip (Video 1, available online at www.giejournal.org).…”

“…The simple application of current endoclips enables only a single-layer mucosal approximation. Using two endoscopes to provide layer-by-layer endoscopic clip closure is another alternative that uses current endoscopic instruments (2). An original technique, using a modified cardiac septal occluder, has demonstrated a zero leak rate (3) and was widely used for survival studies on animal models (Figure 1).…”

“…Original techniques using a cardiac septal occluder(9), surgical stapler(10), transparietal tags(11), bioabsorbable plugs(12) were described. Using two endoscopes to provide layer‐by‐layer endoscopic clip closure is another alternative that uses current endoscopic instruments(13).…”
