Peroral Direct Cholangioscopy (PDCS) Using Routine Straight-view Endoscope: First Report

Abstract: A new endoscopic method, the peroral direct cholangioscopy (PDCS) is described. A prograde fiberscope of 8.8 mm diameter can be directly inserted, without using a second scope as a guide, into the biliary system after EPT (endoscopic papillotomy). The lumen of the common bile duct is observed entirely and exactly. The image is excellent. Also acessories may be inserted into the duct via the biopsy channel under direct control.

“…In 1970, Rosch et al [1] and Urakami [2] independently described two different endoscopic methods for peroral cholangioscopy. Since then, several systems have been introduced; however, widespread adoption of POCPS has been hindered by technological obstacles until recently [3].…”

“…This procedure directly investigates the biliary tract endoscopically and benefits from directly assessing the mucous membrane so as to help evaluate the dignity of inconclusive lesions in the bile duct [9] . For the first time, this offered diagnostic options superior to other imaging procedures in this region [8,9] . In choledochoscopy, a general distinction is made between percutaneous transhepatic and retrograde endoscopic access using the so-called "mother-baby" endoscope technique [10,11] .…”

“…Since the 1970s, choledochoscopy is used mainly in centers focussing on hepatobiliary diagnostics to macroscopically diagnose bile duct lesions [8] . This procedure directly investigates the biliary tract endoscopically and benefits from directly assessing the mucous membrane so as to help evaluate the dignity of inconclusive lesions in the bile duct [9] .…”

“…Peroral cholangioscopy (POCS) was initially described in the mid-1970's. One of the first reports demonstrated that a fiberscope of 8.8 mm diameter could be directly inserted through the mouth, into the biliary system after an endoscopic papillotomy, without the need of using a second scope as a guide [5] . This scope did provide a biopsy channel to obtain tissue samples.…”
